Resumo: The health system in Paraguay does not guarantee full coverage in treatment for people with chronic kidney disease (CKD). There is an important demand from foreign patients with CKD who seek health care in Brazil. Thus, questions arose as to how this access was and in which places these subjects passed to reach the nephrology service in Brazil. The purpose of this study was: To characterize the epidemiological profile of patients of Paraguayan origin in hemodialysis treatment in Foz do Iguaçu - Pr; and Describe the course taken by the foreign chronic renal patient in search of access to treatment of CKD by SUS in Brazil, in the border region. The research was characterized as exploratory and descriptive, which used quantitative and qualitative analysis procedures of the data collected. The epidemiological profile of the patients was identified based on information obtained from the service database. The trajectory was identified through semi-structured interviews, and analyzed from the Kleinman framework that advocates the interference of three subsystems in the Therapeutic Itinerary traveled by people in search of a solution to health problems: family, popular and professional. Total of 23 Paraguayan patients: 73.9% of males; 65.2% are married, 56.5% live with children and 65.2% with their spouse, 26.1% incomplete elementary school, 43.45% between 40 and 59 years and 47.85% with age ≥ 60 years , 52.1% presented with 2 to 5 years of dialysis, the use of catheter as first access occurred in 73.9% of Paraguayans, and 78.3% of them performed the first session of emergency hemodialysis, DM and SAH were the major underlying diseases, 39.2% were not under the care of the nephrologist specialist in the non-dialysis phase. In the family subsystem the subjects and their relatives showed little perception about their disease, and the support and support at the time the subject received the diagnosis of kidney disease is demonstrated by the relatives. In the popular subsystem there was demand for other unconventional care of traditional medicine. In the professional subsystem the subject in the search for his treatment demonstrates throughout his journey how lost and manipulated he is in the health system of his country, often directing him to territories where there is no assistance that he needs or makes it impossible for him to adhere to the treatment. It was concluded that although the hemodialysis clinic was the most frequent site of the first hemodialysis session, the first type of vascular access was the catheter and the hemodialysis session occurred as an emergency in a significant percentage among the Paraguayan patients. In the light of the above, the chronic renal patients traveled in search of care through health care subsystems according to Kleinman's theoretical framework. It should be emphasized that in Brazil there is a public health system based on the principles of universality, completeness, and equity, while in Paraguay the public health system is precarious with low coverage of health care.

Hay una demanda importante de pacientes extranjeros con DRC que buscan asistencia a la salud en Brasil. De esta forma surgieron indagaciones de cómo fue ese acceso y en qué lugares esos sujetos pasaron para llegar hasta el servicio de nefrología en Brasil. Para ello se objetivó: Caracterizar el perfil epidemiológico de pacientes de origen paraguaya en tratamiento de hemodiálisis en Foz do Iguazu - Pr; y Describir el recorrido recorrido por el paciente renal crónico extranjero, en busca de acceso al tratamiento de DRC por el SUS en Brasil, en región de frontera. La investigación se caracterizó como exploratoria y descriptiva, que utilizó procedimientos de análisis cuantitativos y cualitativos de los datos recolectados. El perfil epidemiológico de los pacientes fue identificado con base en informaciones obtenidas en la base de datos del servicio. El recorrido recorrido fue identificado por medio de entrevistas semiestructuradas, y analizado a partir del referencial de Kleinman que defiende la interferencia de tres subsistemas en el Itinerario Terapéutico recorrido por las personas en la búsqueda de solución para problemas de salud: familiar, popular y profesional. Total de 23 pacientes paraguayos: 73,9% del sexo masculino; El 65,5% de los encuestados, el 56,5% de los niños y el 65,2% con el cónyuge, el 26,1% de la enseñanza primaria incompleta, el 43,45% entre 40 a 59 años y el 47,85% con edad ≥ 60 años , El 52,1% presenta de 2 a 5 años de tratamiento dialítico, el uso de catéter como primer acceso ocurrió en el 73,9% de los paraguayos, y el 78,3% de ellos realizaron la primera sesión de hemodiálisis en carácter de emergencia, la DM y la HAS fueron las principales enfermedades de bases, el 39,2% no estaban bajo cuidados del especialista nefrologista en la fase no dialítica. En el subsistema familiar los sujetos y sus familiares presentaron poca percepción acerca de su enfermedad, y el apoyo y soporte en el momento que el sujeto recibió el diagnóstico de la enfermedad renal es demostrado por los familiares. En el subsistema popular hubo demanda por otros cuidados no convencionales de la medicina tradicional. En el subsistema profesional el sujeto en la búsqueda de su tratamiento demuestra en todo su recorrido lo perdido y manipulado se encuentra por el sistema de salud de su país, muchas veces dirigiéndolo hacia territorios donde no hay la asistencia que necesita o le imposibilita la adherencia al tratamiento. Se concluye que aunque la clínica de hemodiálisis fue el lugar de mayor ocurrencia de la primera sesión de hemodiálisis, el primer tipo de acceso vascular fue el catéter y la sesión de hemodiálisis ocurrió en carácter de emergencia en porcentual significativo entre los pacientes paraguayos. Ante los relatos expuestos quedaron destacados por qué caminos los enfermos renales crónicos recorrieron en la búsqueda por su cuidado a través de los subsistemas de atención en salud conforme al referencial teórico de Kleinman. Se resalta que en Brasil existe un sistema de salud pública basado en los principios de la universalidad, integralidad y equidad, mientras que en Paraguay el sistema de salud pública es precario presentando baja cobertura de asistencia a la salud.O sistema de saúde do Paraguai não garante a cobertura total no tratamento para pessoas com doença renal crônica (DRC). Existe uma demanda importante de pacientes estrangeiros com DRC que buscam assistência à saúde no Brasil. Dessa forma, surgiram indagações de como foi esse acesso e quais os caminhos que esses sujeitos percorreram para chegar até o serviço de nefrologia no Brasil. Para tanto, objetivou-se, com este estudo caracterizar o perfil epidemiológico de pacientes de origem paraguaia e brasileira em tratamento de hemodiálise em Foz do Iguaçu – PR e descrever o percurso percorrido pelo doente renal crônico estrangeiro, em busca de acesso ao tratamento de DRC pelo SUS no Brasil, em região de fronteira. A pesquisa caracterizou-se como exploratória e descritiva, utilizando-se procedimentos de análise quantitativos e qualitativos dos dados coletados. O perfil epidemiológico dos pacientes foi identificado com base em informações obtidas no banco de dados do serviço. O percurso percorrido foi identificado por meio de entrevistas semiestruturadas e analisado a partir do referencial de Kleinman, que defende a interferência de três subsistemas no Itinerário Terapêutico percorrido pelas pessoas na busca de solução para problemas de saúde: familiar, popular e profissional. O total de participantes do estudo foram 23 pacientes paraguaios, sendo eles: 73,9% do sexo masculino; 65,2% são casados, 56,5%, os brasileiros 56,45% (n=148) sobre com quem vivem os filhos e 65,2% paraguaios e brasileiros 65,2% (n=15) com o cônjuge, 26,1% ensino fundamental incompleto, 43,45% entre 40 a 59 anos e 47,85% com idade ≥ 60 anos, entre os brasileiros 51,5% (n=135) encontram-se entre 50 a 65 anos de idade 52,1% apresentam de 2 a 5 anos de tratamento dialítico, o uso de cateter como primeiro acesso ocorreu em 73,9% dos paraguaios, e 78,3% deles realizaram a primeira sessão de hemodiálise em caráter de emergência, a DM e a HAS foram as principais doenças de bases, 39,2% não estavam sob os cuidados de nefrologista na fase não dialítica. No subsistema familiar, os sujeitos e seus familiares apresentaram pouca percepção acerca de sua doença, e o apoio e suporte no momento que o sujeito recebeu o diagnóstico da doença renal é demostrado pelos familiares. No subsistema popular, houve procura por outros cuidados não convencionais da medicina tradicional. No subsistema profissional, o sujeito na busca do seu tratamento demostra em todo seu percurso o quão perdido ele se encontra pelo sistema de saúde do seu País, muitas vezes direcionando ele para territórios onde não há a assistência que necessita ou lhe impossibilita a aderência ao tratamento. Conclui-se que, embora a clínica de hemodiálise tenha sido o local de maior ocorrência da primeira sessão de hemodiálise, o primeiro tipo de acesso vascular foi o cateter, e a sessão de hemodiálise ocorreu em caráter de emergência em percentual significativo entre os pacientes paraguaios. Diante dos relatos expostos ficaram destacados por quais caminhos os doentes renais crônicos percorreram na busca pelo seu cuidado por meio dos subsistemas de cuidados em saúde, conforme o referencial teórico de Kleinman. description The health system in Paraguay does not guarantee full coverage in treatment for people with chronic kidney disease (CKD). There is an important demand from foreign patients with CKD who seek health care in Brazil. Thus, questions arose as to how this access was and in which places these subjects passed to reach the nephrology service in Brazil. The purpose of this study was: To characterize the epidemiological profile of patients of Paraguayan origin in hemodialysis treatment in Foz do Iguaçu - Pr; and Describe the course taken by the foreign chronic renal patient in search of access to treatment of CKD by SUS in Brazil, in the border region. The research was characterized as exploratory and descriptive, which used quantitative and qualitative analysis procedures of the data collected. The epidemiological profile of the patients was identified based on information obtained from the service database. The trajectory was identified through semi-structured interviews, and analyzed from the Kleinman framework that advocates the interference of three subsystems in the Therapeutic Itinerary traveled by people in search of a solution to health problems: family, popular and professional. Total of 23 Paraguayan patients: 73.9% of males; 65.2% are married, 56.5% live with children and 65.2% with their spouse, 26.1% incomplete elementary school, 43.45% between 40 and 59 years and 47.85% with age ≥ 60 years , 52.1% presented with 2 to 5 years of dialysis, the use of catheter as first access occurred in 73.9% of Paraguayans, and 78.3% of them performed the first session of emergency hemodialysis, DM and SAH were the major underlying diseases, 39.2% were not under the care of the nephrologist specialist in the non-dialysis phase. In the family subsystem the subjects and their relatives showed little perception about their disease, and the support and support at the time the subject received the diagnosis of kidney disease is demonstrated by the relatives. In the popular subsystem there was demand for other unconventional care of traditional medicine. In the professional subsystem the subject in the search for his treatment demonstrates throughout his journey how lost and manipulated he is in the health system of his country, often directing him to territories where there is no assistance that he needs or makes it impossible for him to adhere to the treatment. It was concluded that although the hemodialysis clinic was the most frequent site of the first hemodialysis session, the first type of vascular access was the catheter and the hemodialysis session occurred as an emergency in a significant percentage among the Paraguayan patients. In the light of the above, the chronic renal patients traveled in search of care through health care subsystems according to Kleinman's theoretical framework. It should be emphasized that in Brazil there is a public health system based on the principles of universality, completeness, and equity, while in Paraguay the public health system is precarious with low coverage of health care.
