Assessment is a fundamental part of the teaching and learning process, and the choice of                    appropriate assessment instruments allows for a more accurate inference of the outcome of this process. In this context, self-assessment can be an alternative to promote a formative assessment that indicates to the student how their learning process is being developed and to the teacher it can be an element for choosing paths to be followed in the continuity of the teaching methodology. This work aimed to develop a self-assessment questionnaire in the search for an alternative for formative assessment. For this, a questionnaire          was built with questions about practice and study methods that relate to the STEM disciplines - Science, Technology, Engineering and Mathematics. This questionnaire was applied to 34 high school students in the year 2022 of a technical school. The data obtained were used to reflections on the teaching practice and students' study. The resulting educational product - methodology for the creating of a self-assessment questionnaire for STEM subjects - is available to the academic and professional community, in the EDUCAPES repository.
