Sarcopenia em pessoas idosas: concordância entre métodos preditores

Resumo: Sarcopenia is a relevant clinical condition worldwide. Its prevalence ranges from 5% to 13% in the elderly aged 65 years and over. Over 80 years, this value can reach 50%, demonstrating the influence of age on the development of the disease. Its diagnosis is based on reduced muscle mass, associated with decreased strength and/or low muscle function. The gold standard for diagnosing the disease is Dual-energy X-ray absorptiometry, but it is inaccessible due to its high cost and diagnostic complexity. Thus, alternative methods for screening for sarcopenia, such as: sit and stand test (TSL), calf circumference (CP) and manual strength test (FPM), SARC-F questionnaire are applied. Defining methods suitable for the outpatient setting is essential for both screening and diagnosis of the disease. The present study aimed to analyze whether there is agreement between predictive methods used to estimate the existence of sarcopenia in the elderly. This is a cross-sectional study carried out with 27 healthy elderly people (23 women and 4 men). The predictive methods were measuring CP, FPM, TSL and Ishii Probability Score (ISHII). The agreement between the classifications was used the Kappa statistic. All predictive methods identified sarcopenic and non-sarcopenic individuals. There was no agreement in the combinations: CTS x FPM; TSL x FPM and; TSL x ISHII. As for the association between the CTS and TSL methods, there was a moderate agreement between the predicted values  (k=0.43; p=0.006). Conclusion: Different predictive methods promote different sarcopenia diagnoses. A moderate agreement was observed for the combination between CTS and TSL predictors. It should be noted that the association between different tests is an option to be evaluated.

Sua prevalência varia de 5% a 13% em idosos com 65 anos ou mais. Acima dos 80 anos, esse valor pode chegar a 50%, demonstrando a influência da idade no desenvolvimento da doença. Seu diagnóstico tem como critérios a redução da massa muscular, associada à diminuição da força e/ou à baixa função muscular. O padrão-ouro para o diagnóstico da doença é o Dual-energy X-ray absorptiometry, mas é pouco acessível devido ao seu elevado custo e complexidade propedêutica. Assim, métodos alternativos para triagem da sarcopenia, tais como: teste de sentar e levantar (TSL), circunferência da panturrilha (CP) e teste de força manual (FPM), questionário SARC-F são aplicados. Definir métodos adequados para o contexto ambulatorial é essencial tanto para o rastreamento quanto para o diagnóstico da doença. O presente estudo teve como objetivo analisar se há concordância entre métodos preditores usados para estimar a existência de sarcopenia em idosos. Trata-se de um estudo de delineamento transversal realizado com 27 idosos saudáveis (23 mulheres e 4 homens). Os métodos preditivos foram a medição CP, FPM, TSL e Escore de Probabilidades de Ishii (ISHII). A concordância entre as classificações foi utilizada a estatística Kappa. Todos os métodos preditivos identificaram indivíduos sarcopênicos e não-sarcopênicos. Não houve concordância nas combinações: CTS x FPM; TSL x FPM e; TSL x ISHII. Já para a associação entre os métodos CTS e TSL houve uma concordância moderada entre os valores preditos (k=0,43; p=0,006). Conclusão: Diferentes métodos preditivos promovem diferentes diagnósticos de sarcopenia. Observou-se uma concordância moderada para a combinação entre os preditores CTS e TSL. Salienta-se que, a associação entre diferentes testes mostra-se como uma opção a ser avaliada.Universidade de São Paulo.
