Resumo: To describe a safe technique for alar base reduction, by resecting the skin, thus reducing the inter-alar distance and enhancing nasal harmony within the face. Case Details: Surgical techniques have evolved considerably over the years, and the same could be said about Alectomy, which aims at reducing the width of the alar base. The main objective was to present the result of a clinical case of Alectomy performed using the Alecgrama®️ technique (diagram of the reduction of the nasal wings without compromising the aesthetics and function of the nose), creating a more harmonious profile for the patient. The methodology applied consisted of a detailed description of the clinical case, wherein the procedure itself was performed with almost no risk, with the benefits of indirectly contributing to a group of individuals who are in the same situation. Aspects not yet observed in the literature were addressed. A Bibliographic methodology was used, as well as a review of the relevant literature based on MEDLINE, EMBASE, LILACS, and SciELO searches. Conclusion: The surgical technique of Alecgrama®️ proved to be effective in meeting the objectives of the surgery and the patient's requests. Nasal reconstruction through alar base reduction, using an appropriate and minimally invasive procedure, met expectations and was consistent with the functional and aesthetic result, showed proper healing and no inflammation process, despite the complexity of the nasal structure.

Detalles del caso: A lo largo de los años, las técnicas quirúrgicas han evolucionado mucho, y no ha sido diferente con la alectomía, cuya finalidad es disminuir el ancho de la base alar. El objetivo principal fue presentar el resultado de un caso clínico de alectomía realizado utilizando la técnica Alecgrama®️ de Algarve & Oliveira (diagrama de reducción de las alas nasales sin comprometer la estética ni la función de la nariz), dejándola más armoniosa con respecto al perfil del paciente. La metodología se realizó describiendo detalladamente el caso clínico, casi sin riesgo del procedimiento en sí, con los beneficios de contribuir indirectamente a un grupo de individuos que se encuentran en la misma situación. Se enumeraron aspectos aún no observados en la literatura. La metodología bibliográfica se utilizó revisando la literatura relevante basada en búsquedas usando MEDLINE, EMBASE, LILACS y SciELO. Conclusión: La técnica quirúrgica Alecgrama®️ demostró ser efectiva para cumplir los objetivos de la cirugía y las solicitudes del paciente. La reconstrucción nasal reduciendo la base alar, usando un procedimiento apropiado y mínimamente invasivo, satisfizo las expectativas y correspondió bien al resultado funcional y estético, sin procesos inflamatorios y con cicatrización adecuada, incluso ante la complejidad de la estructura nasalDescrever uma técnica segura para redução de base alar, através ressecção da pele, diminuindo a distância inter-alar melhorando a harmonia nasal no conjunto facial. Detalhamentos de Caso: Ao longo dos anos as técnicas cirúrgicas têm evoluído muito, e não é o contrário com a Alectomia, cujo propósito é diminuição de largura da base alar. O objetivo principal foi apresentar o resultado de um caso clínico de Alectomia feito através da técnica de Alecgrama® de Algarve & Oliveira (diagrama de redução das asas nasais sem comprometer a estética e função do nariz), deixando-o mais harmônico com o perfil do paciente. Foi realizada a metodologia com a descrição detalhada do caso clínico realizado, com os benefícios de contribuir indiretamente para um grupo de indivíduos que se encontra na mesma situação. Foram relacionados aspectos ainda não observados na literatura. A metodologia bibliográfica foi utilizada com revisão da literatura pertinente com base em buscas através do MEDLINE, EMBASE, LILACS e SciELO. Conclusão: A técnica cirúrgica do Alecgrama®, mostrou-se eficaz para atender os objetivos da cirurgia e dos pedidos da paciente. A reconstrução nasal através da diminuição da base alar utilizando-se procedimento adequado e minimante invasivo, atingiu as expectativas e foi condizente com o resultado funcional e estético, sem processo de inflamação e com cicatrização apropriada, mesmo com a complexidade da estrutura nasal.Profa.
