Quality of life of patients after liver transplantation in a reference transplant center in the interior of São Paulo State

Resumo: Liver transplantation is the therapy indicated for patients with severe and progressive liver disease, whether chronic or acute, who do not benefit from other alternative treatment. Thus, it is the only effecti-ve therapeutic measure capable of significantly modifying the quality of life. Every process experienced by the patient, through the transition from one condition to another, involves factors that impact the declinein quality of life, such as physical symptoms and functional status associated with psychological and social factors, limitations and changes in lifestyle due to the need to adapt to the new reality in which they find themselves. Objective: This study aims to identify the interferences of greater impact on health-related quality of life after undergoing transplantation. Methodology: This is a retrospective, exploratory and des-criptive cohort study, with a qualitative approach. Patients who underwent liver transplantation at Hospital Doutor Miguel Soeiro (HMS) - Unimed Sorocada, from 2005 to 2022 participated in the study. For data collection we used the Short Form Health Survey 36 (SF-36) standardized questionnaire - validated and adapted in Portuguese version - and clinical variables form, with current age, age at transplantation, most common symptoms of liver disease, and feelings experienced in the pre- and post-transplantation process. Results: The Physical Health Component presented the highest means of quality of life in the Functional Capacity (92.4) and Pain (93.22) domains, while the Mental Health Component presented the highest me-ans of quality of life in the Social Aspects (88.69) and Vitality (81.1) domains. It was noteworthy that, the variable 2 - 4 years, showed higher mean scores of quality of life in FSC (89.05), and the variable 1 year, showed higher mean scores in MHSC (90.94). To the others, there were no significant differences, maintai-ning the same pattern among the averages. The approach is by the clinical variables and the feelings ex-pressed by the participants throughout the process. Conclusion: The improvement in QL is proportional to the time of transplantation, due to having to adapt to the new reality in which they find themselves, they recognize the post-transplant benefits and give little or no importance to unforeseen clinical events, as they attribute a new and positive meaning to life after transplantation.

Desta forma, é a única medida terapêutica efetiva, capaz de modificar significativamente a qualidade de vida. Todo processo vi-venciado pelo paciente, pela transição de uma condição para outra, envolve fatores que impactam no de-clínio da qualidade de vida, como, os sintomas físicos e o estado funcional associado a fatores psicológicos e sociais, limitações e mudanças no estilo de vida pela necessidade de adaptação da nova realidade em que se encontra. Objetivo: Este estudo tem a finalidade de identificar os interferentes de maior impacto na qualidade de vida relacionada à saúde após serem submetidos ao transplante. Metodologia: Trata-se de um estudo de coorte retrospectivo, exploratório e descritivo, com abordagem qualitativa. Participaram do es-tudo os pacientes submetidos ao transplante de fígado no Hospital Doutor Miguel Soeiro (HMS) – Unimed Sorocaba, no período de 2005 a 2022. Utilizou-se para a coleta de dados o questionário padronizado Short Form Health Survey 36 (SF-36) - validado e adaptado na versão portuguesa, e formulário de variáveis clíni-cas, com idade atual, idade em que realizou o transplante, sintomas mais comuns da doença hepática, e sentimentos vivenciados no processo de pré e pós transplante. Resultados: O Componente de Saúde Física apresentou maiores médias de qualidade de vida nos domínios de Capacidade Funcional (92,4) e Dor (93,22), enquanto o Componente de Saúde Mental apresentou maiores médias de qualidade de vida nos domínios de Aspectos Sociais (88,69) e Vitalidade (81,1). Destacou-se que, a variável 2 – 4 anos, apresentou maiores médias de score de qualidade de vida em CSF (89,05), e a variável 1 ano, apresentou maiores mé-dias em CSM (90,94). Aos demais, sem diferenças significativas, mantendo um mesmo padrão entre as mé-dias. A abordagem são pelas variáveis clínicas e pelos sentimentos expressados pelos participantes durante todo o processo vivenciado.
