Laryngeal Granuloma: Case Report

Resumo: Introduction: Granulomas are lesions that occur in the region of the vocal process of arytenoid cartilage. Their main causes are severe speech and forced trauma phonation. Chronic laryngeal  irritants, such as gastroesophageal reflux disease, alcohol and tobacco tend to favor their emergence. The treatment is made according to the etiology.Objective: To report a case of contact granuloma, emphasizing its etiology and factors that favor its appearance and discussion about the best treatment.Methods: Review of medical records and clinical follow.Case Report: A 35-year-old man came to the Otorhinolaryngology service presenting with a nuisance complain in the throat that worsened with exertion. We conducted a videolaryngoscopy which revealed a laryngeal granuloma. After this diagnosis he was submitted to a surgery to remove the granuloma, but the patient still had a nuisance complain in the throat after the procedure. Then, he was re examined by videolaryngoscopy and endoscopy, followed by treatment for gastroesophageal reflux, with recovery..Conclusion: Laryngeal granuloma diagnosis is made by clinical history, examination and complementary tests. On clinical history, patients can present a variety of symptoms, from a mild dysphonia and severe, pain in the larynx, constant throat clearing, reflex otalgia, oreign body sensation in the throat etc. These symptoms worsen with vocal effort. Videolaryngoscopy is the best examination and surgery is sometimes necessary. The cause of granuloma should always be investigated to remove it and prevent recurrences.

Suas principais causas são a fonação intensa e forçada com trauma fonatório, intubação endotraqueal e refluxo gastroesofágico. Irritantes crônicos da laringe, como álcool e tabaco tendem a favorecer seu surgimento. O tratamento faz-se de acordo com a etiologia.Objetivo: Relatar um caso de granuloma de contato, enfatizando sua etiologia e fatores que favorecem o seu surgimento e discussão sobre o melhor tratamento.Método: Revisão de prontuário e acompanhamento clínico.Relato de Caso: Homem, de 35 anos de idade, procurou o serviço de Otorrinolaringologia por apresentar um "incômodo" na garganta que piorava com esforço. Foi realizada uma videolaringoscopia que evidenciou um granuloma laríngeo. Após esse diagnóstico foi submetido à cirurgia para remoção do granuloma, porém o paciente ainda apresentava "incômodo" na garganta após o procedimento. Foi então reavaliado através de novo exame de vídeolaringoscopia e endoscopia digestiva alta (EDA) seguido de tratamento para refluxo gastroesofágico, evoluindo com melhora.Conclusão: O diagnóstico de granuloma laríngeo é feito pela história clínica, pelo exame otorrinolaringológico e exames complementares. Na história clínica os pacientes podem apresentar desde uma disfonia leve a acentuada, dor na laringe, pigarrear constante, otalgia reflexa, sensação de corpo estranho na garganta, etc. Estes sintomas pioram com o esforço vocal. A videolaringoscopia é o melhor exame e a cirurgia algumas vezes é necessária.
