FACTORS RELATED TO BACTERIAL RESISTANCE IN PATIENTS HOSPITALIZED IN A HIGH-COMPLEXITY SCHOOL HOSPITAL IN THE STATE OF RIO DE JANEIRO

Resumo: Objective: To identify factors related to bacterial resistance in the hospital environment. Methods: Analysis of 69 medical records of patients who presented bacterial resistance in inpatient units of a teaching hospital in March, April and May 2018. A cross-sectional observational study.The study variables are age, gender, length of stay, primary diagnosis of hospitalization, inpatient unit, internal and external transfer, previous exposures, risk factors for colonization, invasive devices, antibiotic therapy, culture microbiology. The research was registered in Plataforma Brasil and approved with opinion number 2,483,545. Results: The predominant age group was over 61 years old, male (53.3%), undergoing cancer treatment (30%), with hospitalization time over 15 days. The use of invasive devices was observed in 92% of patients. The highest percentages of microorganisms resistance were observed with sulfonamides, tetracycline and clindamycin. The most commonly prescribed antibiotics for the treatment of resistance cases were piperacilna + tazobactan, ceftriaxone and ciprofloxacin. Conclusion: The methodology employed identified that the main factors related to bacterial resistance at the study site were patients undergoing cancer treatment, length of stay longer than fifteen days, risk factors such as hypertension, vascular diseases, chemotherapy and invasive procedures.

Métodos: Análise de 69 prontuários de pacientes que apresentaram resistência bacteriana em unidades de internação de um hospital escola nos meses de março, abril e maio de 2018. Estudo do tipo observacional transversal. As variáveis de estudo são idade, sexo, período de internação, diagnóstico principal de internação, unidade de internação, transferência interna e externa, exposições prévias, fatores de risco para colonização, dispositivos invasivos, antibioticoterapia, microbiologia das culturas. A pesquisa foi registrada na Plataforma Brasil e aprovada com o parecer número 2.483.545. Resultados: A faixa etária predominante foi acima de 61 anos, do sexo masculino (53,3%), em tratamento oncológico (30%), com tempo de internação acima de 15 dias. O uso de dispositivos invasivos foi observado em 92% dos pacientes. Os maiores percentuais de resistência dos microorganismos foram observados com as sulfonamidas, tetraciclina e clindamicina. Os antibióticos mais prescritos para o tratamento dos casos de resistência foram a piperacilna + tazobactan, ceftriaxona e ciprofloxacino.
