Malignant and Premalignant Skin Lesions After Kidney Transplant: case report and literature review

Resumo: In its function of mediator between the organism and the environment, the skin, besides its barrier fuction, also presents immunological defense mecanisms. In kidney tranplants, these protection mecanisms are harmed by the use of immunosuppresives. Objective: Report a case of multiple squamous cell carcinomas and actinic keratoses, besides common warts and porokeratosis, in a eight year kidney transplanted patient. Description: Female patient, 49 years old, white, skin type II, countryside resident, kidney transplanted 15 years ago, presents multiples actinic keratosis especially in upper limbs and in the face. The patient initially used triple regimen of immunosuppression with Azathioprine 120mg/ day, Predinisone 20mg/day and Tacrolimus 5mg/twice a day. Approximately eight years after the transplant, multiple actinic keratosis appeared and there were diagnosed multiple carcinomas in photo-exposed areas. As a treatment conduct, it was scheduled a cryotherapy in order to treat the actinic keratosis, besides surgical removal of suspicious malignant lesions. Conclusions: It is extremely important the dermatological exam and the periodic monitoring in all the kidney transplanted patients, which allows the early diagnostic and treatment not only of nonmelanoma skin carcinomas, but also of multiple pre-malignant existing lesions.

Nos transplantados renais, esses mecanismos de proteção estão prejudicados pelo uso de imunossupressores. Objetivo: Relatar um caso de múltiplos carcinomas espinocelulares e ceratoses actínicas, além de verrugas vulgares e poroceratose, em uma paciente após 8 anos de transplante renal. Descrição: Paciente feminina, 49 anos, branca, fototipo II, moradora de zona rural, transplantada renal há 15 anos, fez uso inicialmente de esquema tríplice de imunossupressão com Azatioprina 120mg/dia, Prednisona 20mg/dia e Tacrolimus 5mg/2 vezes ao dia. Aproximadamente 8 anos após o transplante, ocorreu o aparecimento de múltiplas ceratoses actinicas principalmente em membros superiores e face e múltiplos carcinomas espinocelulares em áreas fotoexpostas foram diagnosticados. Como conduta foi agendada crioterapia para tratamento das lesões de ceratoses actinícas, além de retirada cirúrgica de lesões suspeitas de malignidades.
