Resumo: Syphilis is an infectious disease of great incidence and epidemiological importance, and actions aimed at encouraging safe sexual practices can contribute to modify this statistic. Congenital syphilis is characterized as the infection of the fetus, when the pregnant woman with the infection is not treated or receives inadequate treatment. The purpose of this report is to describe a case of renal manifestation of Syphilis in a 3-month-old patient, who presented syndromic and laboratory manifestations compatible with nephrotic syndrome, and their challenges for management and diagnostic conclusion. Given this situation, it is necessary to raise the awareness of professionals and the community about the importance of early diagnosis and effective treatment.

A Sífilis congênita é caracterizada como a infecção do concepto, quando a gestante portadora da infecção não é tratada ou recebe tratamento inadequado. O objetivo desse relato é descrever um caso de manifestação renal da sífilis em paciente de 3 meses, que apresentou manifestações sindrômicas e laboratoriais compatíveis com síndrome nefrótica, e seus desafios para o manejo e a conclusão diagnóstica.
