Resumo: This article describe a case study of a patient who suffered a chemical burn on the left hallux. The patient, who had onychocryptosis, was treated with phenol acid, which led to necrosis, foul odor, and purulent secretion. Upon medical evaluation, a third-degree infected burn was diagnosed, and initially, the patient underwent non-surgical treatment. Eventually, a Keystone skin flap was performed to close the lesion, and the patient fully recovered after two months. The study concludes that using acids can result in severe chemical burns, compromising the function and viabilityof the affected digit. Furthermore, in deep wounds, Keystone flap represents a good local alternative with a rapid recovery rate.

Paciente com onicocriptose foi submetida a tratamento com ácido fênico, evoluindo com necrose, odor fétido e secreção purulenta. Após avaliação médica, diagnosticou-se queimadura de terceiro grau infectada, sendo submetida, a princípio a abordagem não cirúrgica, e posteriormente a realização de retalho cutâneo Keystone para fechamento da lesão com recuperação completa após dois meses.
