Epidemiological profile of congenital syphilis cases in the state of Piauí

Resumo: Introduction: Syphilis is a systemic infection that can be transmitted through sexual intercourse, the placenta or blood transfusions. It is a preventable disease, but when not treated early, it can develop into a chronic condition with irreversible sequelae. Once transmission occurs via the placenta, it results in a disease called congenital syphilis. Objective: To analyze the epidemiological profile of congenital syphilis cases in the state of Piauí. Methodology: This is an epidemiological, retrospective and analytical study carried out by analyzing cases of congenital syphilis in the state of Piauí. The data was obtained from the Notifiable Diseases Information System (SINAN) via DATASUS. In order to deepen the research, the following variables were studied: year of notification, IBGE micro-region of residence according to the year of diagnosis, color/race, prenatal care, maternal age and schooling, and case progression. Results and discussion: in the time frame analyzed, 1,665 cases were reported, with the highest occurrences in 2018 (499 cases), 2017 (443) and 2019 (396 cases). Still in the annual context, we can see a variation with a decline from 2020 onwards. Regarding the municipalities of notification, the capital Teresina stood out with the highest rates of notifications (1,095 occurrences). The main characteristics related to the mother were age between 20 and 29 years (50.93%), whose color/race was mostly brown (69.6%) and associated with low schooling (25.89% had not completed elementary school). Conclusion: Even in the face of various current methods that can be used to circumvent the context of congenital syphilis, it is clear that this is still a current and persistent narrative. In this sense, it is understood that the tools, capture, diagnosis and continuity of treatment during prenatal care have gaps that need to be resolved.

Una vez que la transmisión se produce a través de la placenta, da lugar a una enfermedad denominada sífilis congénita. Objetivo: Analizar el perfil epidemiológico de los casos de sífilis congénita en el estado de Piauí. Metodología: Se trata de un estudio epidemiológico, retrospectivo y analítico realizado a partir del análisis de los casos de sífilis congénita en el estado de Piauí. Los datos fueron obtenidos del Sistema de Información de Enfermedades de Declaración Obligatoria (SINAN) vía DATASUS. Para profundizar la investigación, se estudiaron las siguientes variables: año de notificación, microrregión IBGE de residencia según el año de diagnóstico, color/raza, control prenatal, edad y escolaridad materna y evolución de los casos. Resultados y discusión: 1.665 casos fueron notificados en el período analizado, con el mayor número de casos en 2018 (499 casos), 2017 (443) y 2019 (396 casos). Siempre en el contexto anual, se observa una variación con un descenso a partir de 2020. En cuanto a los municipios de notificación, la capital Teresina se destacó con las mayores tasas de notificaciones (1.095 ocurrencias). Las principales características relacionadas a la madre fueron edad entre 20 y 29 años (50,93%), cuyo color/raza era mayoritariamente morena (69,6%) y asociada a baja escolaridad (25,89% no habían concluido la enseñanza primaria). Conclusión: Incluso frente a los diversos métodos actuales que pueden ser utilizados para eludir el contexto de la sífilis congénita, se puede observar que esta sigue siendo una narrativa actual y persistente. En este sentido, se entiende que las herramientas, el cribado, el diagnóstico y la continuidad del tratamiento durante la atención prenatal tienen lagunas que deben ser resueltas.Introdução: A sífilis é uma infecção de caráter sistêmico, sua transmissão pode ser dada através do ato sexual, da placenta ou da transfusão sanguínea. É uma doença que pode ser prevenida, porém quando não tratada precocemente, pode evoluir para um quadro crônico com sequelas irreversíveis. Uma vez ocorrido o meio de transmissão por via placentária, resulta-se em uma doença denominada sífilis congênita. Objetivo: Analisar o perfil epidemiológico de casos de sífilis congênita no estado de Piauí. Metodologia: Trata-se de um estudo epidemiológico, retrospectivo e analítico desenvolvido por meio da análise dos casos de sífilis congênita no estado do Piauí, os dados foram obtidos pelo Sistema de Informação de Agravos de Notificação (SINAN) via DATASUS. Para aprofundamento da pesquisa, foram estudadas as seguintes variáveis: ano da notificação, Microrregião IBGE de residência segundo o ano de diagnóstico, cor/raça realização do pré-natal, faixa etária e escolaridade materna e evolução do caso. Resultados e discussão: no recorte temporal analisado foram notificadas 1.665 casos, deste observa-se maior ocorrências entre os anos de 2018 (499 casos), 2017 (443) e 2019 com 396 casos. Ainda no contexto anual, consegue-se notar uma variação com declínio a partir de 2020. Acerca dos municípios de notificação, foi perceptível maior destaque na capital Teresina com maiores índices de notificações (1.095 ocorrências). As principais características relacionadas à mãe foram a idade de 20 a 29 anos (50,93%), cuja cor/raça afirmadas, em sua maioria eram pardas (69,6%) e associadas à baixa escolaridade (25,89% não possuíam o ensino fundamental concluído). Conclusão: mesmo diante de diversos métodos atuais utilizáveis para contornar o contexto das sífilis congênitas, percebe-se que ainda se trata de uma narrativa atual e persistente.
