Work and practices of obstetric care of nursing in public maternity

Resumo: The way in which work and care practices are organized in institutional dynamics are crucial for the qualification of obstetric care. This study aimed to describe the work and care practices offered by nursing professionals in a public maternity hospital. Qualitative research conducted by the methodological framework of ethnonursing was used, carried out with 12 key informants in a public maternity located in the region of Cariri (state of Ceará). The Observation-Participation-Reflection enabler was adopted for data collection, recording observations in a field diary and using individual interviews. As results, cultural themes emerged focused on work in the maternity hospital (“I love working here, I feel fulfilled;” “I think the team is very worn out;” and “not all the nursing team is engaged with the medical team”) and for the care practices offered (“the bureaucratic part complicates the care part a little;” “during the assistance, the students help and the companions get in the way;” and “the humanization leaves a lot to be desired here in the maternity ward”). In conclusion, the meanings attributed to work and care practices were marked by (dis)satisfaction with structural, relational, labor and work conditions and the management model linked to the entity responsible for managing maternity.

El objetivo de este estudio fue describir las prácticas de trabajo y atención que ofrecen los profesionales de enfermería en una maternidad pública. Se realizó una investigación cualitativa, basada en la etnoenfermería, en la cual participaron 12 informantes en una maternidad pública ubicada en la región de Cariri, en Ceará (Brasil). Se adoptó el habilitador Observación-Participación-Reflexión para la recolección de datos, con registro de las observaciones en un diario de campo y la aplicación de entrevistas individuales. Surgieron temas culturales centrados en el trabajo en la maternidad (“Me encanta trabajar aquí, me siento bastante bien”; “Creo que el equipo está muy desgastado” y “no todo el equipo de enfermería está comprometido con el equipo médico”) y en las prácticas de asistencia ofrecida (“la parte burocrática complica un poco la parte asistencial”; “durante la asistencia los académicos ayudan y los acompañantes estorban” y “la humanización deja mucho que desear aquí en la maternidad”). Se concluye que los significados atribuidos a las prácticas de trabajo y atención estuvieron marcados por la (in)satisfacción con las condiciones estructurales, relacionales, mano de obra y laborales, y con el modelo de gestión vinculado a la entidad responsable de la gestión de la maternidad.A forma como o trabalho e as práticas de cuidado são organizadas nas dinâmicas institucionais são determinantes para a qualificação da assistência obstétrica. O objetivo deste estudo foi descrever o trabalho e as práticas de cuidado ofertadas por profissionais de enfermagem em maternidade pública. Foi utilizada uma abordagem qualitativa, conduzida pelo referencial metodológico da etnoenfermagem, realizada com 12 informantes-chave em uma maternidade pública localizada na região do Cariri (CE). Adotou-se, para coleta de dados, o capacitador Observação-Participação-Reflexão, com registro das observações em diário de campo e uso de entrevistas individuais. Como resultado, emergiram temas culturais voltados para o trabalho na maternidade (“eu adoro trabalhar aqui, me sinto realizada”; “acho que tem um desgaste muito grande da equipe”; e “nem toda a equipe de enfermagem é engajada com a equipe médica”) e para as práticas de cuidado ofertadas (“a parte burocrática complica um pouco a parte da assistência”; “durante a assistência, os acadêmicos ajudam e os acompanhantes atrapalham”; e “a humanização deixa muito a desejar aqui na maternidade”).
