Oral Candida as an Aggravating Factor of Mucositis Induced by Radiotherap

Resumo: Introduction: Antineoplastic treatment induces some undesirable consequences in head and neck cancer patients. Often, the emergence of major clinical manifestations, such as oral mucositis, results in temporary interruption of the treatment, decreasing the patients’ quality of life, and increasing hospital costs. Radio-induced or chemo-induced oral mucositis is possibly aggravated by opportunist fungal infections, which turn the mucositis more resistant to the conventional treatments. Objective: This study aims to identify the presence of Candida sp. as a possible aggravating factor of oral mucositis in patients with head and neck cancer under antineoplastic treatment. Method: All patients with radio- or chemo-induced oral mucositis from the Cancer Hospital of Pernambuco, treated between October 2008 and April 2009, were selected for the study. The prevalence of Candida sp was measured through the cytological analysis of oral mucosa in patients with oral mucositis. The fungal presence was correlated with the mucositis severity. Results: The results showed a positive association between fungal colonization and more several lesions (degrees III and IV of mucositis). Conclusion: The outcomes shown may contribute to a solution for unconventional mucosites, which do not respond to the usual treatment.

Muchas veces, el surgimiento de alteraciones clínicas graves, como las mucositis, obliga a la interrupción temporal del tratamiento, disminuyendo la calidad de vida del paciente y aumentando los costos de su hospitalización. Las infecciones por hongos oportunistas pueden agravar la mucositis bucal por irradiación y quimioterapia, convirtiéndola en una enfermedad más resistente a los tratamientos convencionales. Objetivo: Este trabajo objetiva identificar la presencia de especies de Candidiasis y analizar la posibilidad de este hongo actuar como factor agravante de la mucositis en los pacientes de cáncer de cabeza y cuello, los cuales estén sometidos al tratamiento antineoplásico. Método: Los pacientes fueran elegidos de manera consecutiva en el Hospital de Cáncer de Pernambuco, entre octubre de 2008 hasta abril de 2009. La prevalencia de Candidiasis sp fue mensurada a través la extensión citológica de los pacientes con mucositis bucal. La presencia del hongo fue correlacionada con el grado de severidad de las lesiones de mucositis. Resultados: Los resultados mostraron una asociación positiva entre la colonización de hongos y las lesiones más severas (grados III y IV). Conclusión: Los resultados presentados pueden contribuir para la resolución de mucositis no convencionales, las cuales no responden al tratamiento usual.  Introdução: O tratamento antineoplásico provoca algumas sequelas indesejáveis no paciente com câncer de cabeça e pescoço. Muitas vezes, o surgimento de manifestações clínicas graves, como as mucosites, obriga a interrupção temporária do tratamento, diminuindo a qualidade de vida do paciente e aumentando os custos do seu internamento. É possível que a mucosite oral induzida pela irradiação e quimioterapia seja agravada por infecções fúngicas oportunistas que a torna mais resistente aos tratamentos convencionais. Objetivos: Este trabalho tem como objetivos identificar a presença de espécies de Candida e analisar a possibilidade deste fungo atuar como fator agravante da mucosite em pacientes portadores de câncer de cabeça e pescoço, os quais estejam sendo submetidos ao tratamento antineoplásico. Método: Os pacientes foram selecionados de modo consecutivo no Hospital do Câncer de Pernambuco no período compreendido entre outubro de 2008 a abril de 2009. A prevalência de Candida sp foi mensurada através da análise de raspados citológicos dos pacientes com mucosite oral. A presença do fungo foi correlacionada com o grau de severidade das lesões de mucosite. Resultados: Os resultados mostraram uma associação positiva entre a colonização fúngica e as lesões mais severas (graus III e IV).
