General Aspects of Breast Carcinomas, Staging and Histopathological Classification, with Description of the Main Patterns.

Resumo: Breast cancer is not only the most common malignancy but also the one showing the worst mortality rate in Brazilian women. Multicentricity occurs in about 13% of the cases and is more common in women with a positive family history and in those with lobular carcinoma. In spite of the fact that breast palpation is easy and simple for diagnosing, only about 60% of mammography-positive tumours are palpahle. Anatomopathological analysis is indispensable for making a diagnosis of the primary tumour and the metastases, and for classification and staging. The breast adenocarcinomas are classified as in sim and invasives. These two are then subdivided as ductal and lobular, each one of them having its own subtypes, due to histopathological characteristics which influence the clinical course. Among the ductal carcinoma in situ, comedocarcinomas can reach big sizes and become palpable. These tumours are of high nuclear grade and can be invasive early. Classic invasive ductal carcinoma (IDC), the most common malignancy ofthe breast, shows the worst prognostic rate, mainly those with higher histohgical grade. The IDC is followed in frequency by invasive lobular carcinoma, which prognosis is a little better than IDC.

Ele se localiza mais freqüentemente no quadrante superior externo da mama e em cerca de 13% é multicêntrico, o que é mais comum entre os carcinomas lobulares e entre as mulheres que têm história familiar. Apesar da palpação ser o método mais simples para o diagnóstico, somente cerca de 60% dos cânceres detectados por mamografia são palpáveis. O exame anatomopatológico é imprescindível para o diagnóstico do tumor primário e das metástases e para a classificação e o estadiamento. Os tumores malignos próprios da mama, isto é, os adenocarcinomas, são divididos em in situ e invasores e estes dois subgrupos, por sua vez, são divididos em ductais e lobulares. Cada um destes subgrupos tem seus subtipos, cada qual com suas características histopatológicas próprias que influenciam o comportamento clínico. Dentre os carcinomas ductais in situ, chama atenção o tipo comedocarcinoma que pode atingir grandes dimensões e se tornar palpável. Estes tumores são de alto grau nuclear e podem se tornar invasores precocemente. O carcinoma ductal infiltrante (GDI) clássico, além de tipo de tumor maligno mais comum da mama, é também o de pior prognóstico, principalmente aqueles de alto grau histológico.
