Neoadjuvant Chemotherapy in Operable Breast Cancer: Literature Review

Summary: Introduction: Neoadjuvant chemotherapy was initially used in the systemic treatment of locally advanced and inoperable breast cancer. Based on the results obtained with this therapeutic approach, neoadjuvant treatment started to be offered to patients with operable tumors and a wide experience has already accumulated in this indication. Objectives: To conduct a review of published literature about neoadjuvant or primary chemotherapy in the treatment of breast cancer with special attention to operable breast cancer, trying to summarize the indications, treatment regimens and outcomes. Method: Data were identified through a review of articles published from 1997 to 2012, using PubMed/ Medline (www.ncbi.nlm.nih.gov/pubmed) with the words: "neoadjuvant therapy", "primary systemic therapy", "breast neoplasms" and "operable breast cancer." Results: We found in the medical literature 1,043 trials and selected 25 of them for this review (conducted prospectively, meta-analysis or cohort analysis). Conclusion: After the analysis it can be concluded that in operable breast cancer, neoadjuvant chemotherapy offers survival results equivalent to those obtained with adjuvant chemotherapy, allowing higher percentage of conservative surgery, improvement of surgical outcomes and appropriate assessment of prognosis. Treatment regimens usually employ anthracyclines, taxanes and, when indicated, biological therapy. In the experimental field, it contributes as a basis for rapid development of better and more effective treatments of breast cancer.

Con la base adquirida en esta propuesta terapéutica, se comenzó a realizar el tratamiento neoadyuvante en pacientes con tumores operables y se tuvo una amplia experiencia acumulada en esta indicación. Objetivos: Realizar una revisión de la literatura publicada sobre la quimioterapia neoadyuvante o primaria en el tratamiento del cáncer de mama operable, tratando de resumir las indicaciones, los regímenes de tratamiento y resultados. Método: Los datos fueron identificados a través de la revisión de los artículos publicados desde 1997 hasta 2012, utilizando la herramienta PubMed/ Medline (www.ncbi.nlm.nih.gov.pubmed), con las palabras: “terapia neoadyuvante”, “terapia sistémica primaria”, “neoplasias de la mama” y “cáncer de mama operable”. Resultados: Se encontró en la literatura médica publicada 1.043 ensayos y se utilizaron 25 de ellos (realizados de forma prospectiva, meta-análisis y análisis de cohorte). Conclusión: Se puede concluir que en el cáncer de mama operable, la quimioterapia neoadyuvante ofrece resultados de supervivencia equivalentes a los obtenidos con la quimioterapia adyuvante, lo que permite un mayor porcentaje de cirugía conservadora, la mejora de los resultados quirúrgicos y una adecuada evaluación del pronóstico. Los regímenes de tratamiento más establecidos emplean antraciclinas, taxanos y, cuando este indicado, terapia biológica. En el campo experimental, contribuye como una base para el rápido desarrollo de tratamientos mejores y más eficaces para el cáncer de mama.  Introdução: A quimioterapia neoadjuvante foi inicialmente utilizada no tratamento sistêmico do câncer de mama localmente avançado e inoperável. Com a base adquirida nessa proposta terapêutica, passou-se a realizar o tratamento neoadjuvante em pacientes com tumores operáveis, e uma ampla experiência se acumula nessa indicação. Objetivos: Realizar uma análise crítica da literatura publicada sobre a quimioterapia neoadjuvante ou primária, priorizando sua indicação no tratamento câncer de mama operável. Método: Os dados da revisão foram identificados através de artigos publicados entre 1997 a 2012, tendo como ferramenta o PubMed/MedLine (www.ncbi.nlm.nih.gov/ pubmed), utilizando as palavras-chave: “neoadjuvant therapy”, “primary systemic therapy”, “breast neoplasms” e “operable breast cancer”. Resultados: Foram encontrados na literatura médica, 1.043 ensaios clínicos e utilizados 25 deles nesta revisão (estudos prospectivos, metanálise e análise de coortes). Conclusão: Após a análise, pode-se concluir que, no câncer de mama operável, a quimioterapia neoadjuvante oferece resultados de sobrevida equivalentes aos obtidos com quimioterapia adjuvante, permitindo aumento do percentual de cirurgias conservadoras, melhora dos desfechos cirúrgicos e adequada avaliação do prognóstico. Os esquemas de tratamento mais estabelecidos empregam antraciclinas, taxanes e, quando indicado, terapia biológica.
