Malocclusion, dental caries and perceptions of aesthetics and function: an study of association

Resumo: Assessment of malocclusion is important for documentation of the prevalence and severity of malocclusion in population groups. The purpose of this study was to determine the prevalence of malocclusion and caries in children aged 12 years and to investigate whether a relationship exists between prevalence of caries and studied malocclusion. The relationship between severity of malocclusion and perceptions of aesthetics and function of teeth also was assessed. Eighty-eigth schoolchildren (45 boys and 43 girls) were randomly selected, none of them had received previous orthodontic treatment. One examiner assessed the students using the Dental Aesthetic Index (DAI) for malocclusion and DMFT for dental caries according World Health Organization (WHO). Subjects were also asked to complete a questionnaire consisting of two questions concerning appearance and function of teeth. For statistical analysis chi-square test and Fisher exact were used. No positive correlation between prevalence of caries and malocclusion could be stablished as well as between chewing and severity of malocclusion (p > 0.05). However, statistically significant correlation were found between appearance of teeth (p < 0.01) and scores of DAI. There are different degrees of objectively defined occlusal problems acceptable in the sample of students and that could interfere with their decision to seek treatment and have a direct impact on the need of care. Subjective measures should be incorporated to clinical criteria currently used. UNITERMS: dental caries; malocclusion; epidemiology.

Este estudo objetivou determinar a prevalência das maloclusões e da cárie dentária em crianças de 12 anos de idade e investigar uma possível associação existente entre a prevalência de cárie dentária e as maloclusões observadas. A relação entre a severidade das maloclusões e percepções de estética e função dos dentes também foi avaliada. Oitenta e oito escolares (45 meninos e 43 meninas) foram aleatoriamente selecionados, nenhum deles tinha recebido qualquer tipo de tratamento ortodôntico anteriormente. Um examinador devidamente calibrado avaliou os estudantes usando o Índice de Estética Dental (DAI) para as maloclusões e o CPO-D para a cárie dentária de acordo com critérios da Organização Mundial de Saúde (OMS). Os estudantes também responderam a um questionário contendo duas perguntas relacionadas a aparência e função de seus dentes. Para a análise estatística foi usado o Teste Qui-quadrado e Exato de Fisher. Nenhuma correlação pode ser estabelecida entre a prevalência de cárie e as maloclusões estudadas bem como entre função mastigatória e severidade das maloclusões (p > 0,05). No entanto uma correlação estatisticamente significativa (p < 0,01) pode ser encontrada entre a aparência dos dentes e escores do DAI. Os resultados sugerem que existem diferentes graus de problemas oclusais tecnicamente definidos e que são aceitáveis na amostra dos estudantes e que deveriam influenciar na decisão de tratamento, interferindo diretamente na demanda para esse tipo de atendimento. Medidas subjetivas poderiam ser incorporadas aos critérios clínicos atualmente utilizados.
