Globalisation, ageing and the role of health services

Resumo: The global demographic and epidemiological scenario is dominated by a rapid growth in the incidence of chronic diseases, both in high-income countries and those with medium / low income. The higher incidence of chronic pathologies in a context of increased average life expectancy produces a higher incidence of complex patients, who present the association of several pathologies and/or the co-presence of unfavorable social conditions (health and social vulnerability). This type of patient needs integrated interventions at different levels. The new epidemiological scenario represents a threat to Health Systems. This epidemiological context foresees an active intervention of the primary care sector in the coordination of interventions in the intermediate care sector, with a multi-service integration function between hospital and territory, and multi-sector integration between social, health, community and family resources.Keywords:

La mayor incidencia de patologías crónicas en un contexto de mayor esperanza de vida media produce una mayor incidencia de casos de pacientes complejos, que presentan la asociación de varias patologías y / o la copresencia de condiciones sociales desfavorables (vulnerabilidad social y sanitaria). Este tipo de paciente necesita intervenciones integradas a diferentes niveles. El nuevo escenario epidemiológico constituye una amenaza para los Sistemas de Salud. Tal contexto epidemiológico prevé una intervención activa del sector de atención primaria en la coordinación de intervenciones en el sector de atención intermedia, con una función de integración multiservicio entre hospital y territorio, e integración multisectorial. entre recursos sociales, sanitarios, comunitarios y familiares. O cenário demográfico e epidemiológico global é dominado por um rápido crescimento na incidência de patologias crônicas seja em países de alta renda seja naqueles com renda média ou baixa. A maior incidência de patologias crônicas em um contexto de aumento da expectativa de vida média produz uma maior incidência de casos de pacientes complexos, que apresentam a associação de diversas patologias e/ou a copresença de condições sociais desfavoráveis (vulnerabilidade sociosanitária). Esse tipo de paciente precisa de intervenções integradas em diferentes níveis. O novo cenário epidemiológico representa uma ameaça para os Sistemas de Saúde.
