Os pacientes com Perturbação Dismórfica Corporal são vistos muitas vezes na cirurgia plástica à procura de tratamento cirúrgico. A prevalência desta doença na cirurgia plástica e na população em geral está ainda por determinar. Foram propostos vários testes para diagnosticar este distúrbio, no entanto, uma vez que os cirurgiões estão ,ainda, pouco sensibilizados para esta doença, continua a ser subdiagnosticada nesta área da Medicina. A Perturbação Dismórfica Corporal resulta da combinação complexa de vários fatores neurobiológicos, cognitivos, comportamentais e sociais. Daí a elevada dificuldade de diagnosticar, diferenciar de outras patologias e tratar . Pacientes que sofrem de Perturbação Dismórfica Corporal odeiam o seu defeito e podem procurar a solução na cirurgia plástica. Contudo, estes pacientes ficam invariavelmente insatisfeitos com o resultado. Alguns podem, eventualmente, tornarem-se viciados em cirurgias. Com o objetivo de esquematizar o conhecimento mais recente sobre esta temática foi realizada uma pesquisa na base de dados MEDLINE onde foram identificados os estudos de maior relevo publicados nos últimos vinte anos, sem menosprezar os raros trabalhos clássicos anteriores. Apesar de, nos últimos anos, terem sido feitos progressos relativamente à fisiopatologia, à personalidade e determinação de quais os fatores de risco que podem condicionar o aparecimento da doença, são ainda necessários mais estudos de forma a estabelecer relações inequívocas de causalidade. Estes doentes precisam, principalmente, de serem referenciados a um psiquiatra ou psicólogo clínico especializado. Reconhecer este tipo de doentes e negar a cirurgia são medidas que se impõe ao cirurgião plástico de modo a evitar eventuais transtornos médico-legais. De investigações futuras poderão surgir novas medidas de diagnóstico simples e acessíveis de usar numa consulta de cirurgia plásticaBody Dysmorphic Disorder is a somatoform disorder characterized by an excessive concern from the patient with a very small or even imagined defect in his physical appearance. Patients with Body Dysmorphic Disorder are often seen in plastic surgery consultations seeking surgical treatment. The prevalence of this disease in plastic surgery and in the general population is yet to be determined. Several tests have been proposed in order to diagnose this condition, however, since surgeons are not very aware to this disease it continues to be underdiagnosed in this area of medicine. The Body Dysmorphic Disorder results as a complex combination of several neurobiological, cognitive, behavioral and social factors. Therefore, it’s difficult to diagnose, differentiate from other diseases and treat it effectively. Patients who suffer from Body Dysmorphic Disorder usually hate their body and try to find a solution to their issues with plastic surgery. However these patients stay invariably unsatisfied with the result of the surgery. Some may, eventually, become addicted to surgery. With the purpose to outline the latest knowledge about this subject, a search was conducted in MEDLINE database which identified the most relevant studies published in the last twenty years, but always having in consideration rare previous classic studies. Although, in recent years, progress was made regarding the pathophysiology and determining which risk factors that may influence the onset of the disease, more studies are still needed in order to establish defined causal relationships. These patients need, most importantly, to be referenced to a psychiatrist or a specialized clinical psychologist. To recognize the features of this kind of patient and deny him medical procedures are ways for a surgeon to avoid eventual medical and legal actions against the surgeon.
