Aortic valve abscess – a clinical case

Resumo: The authors present a clinical case of infective endocarditis, involving a 10 year-old prosthetic aortic valve, with perivalvular abscess formation, in a 32 year old patient. The clinical evolution was quite typical, progressing from an insidious febrile syndrome to an acute left ventricular failure. The isolated agent was a Streptococcus mitis, which, in presence of multiple gengivo-dental infections (with poor oral and dental higyene), sugests an oropharingeal primary site of infection.Based on this case, the authors performed a literature revision about prosthetic valve endocarditis, with special incidence on aortic valve. Clinical history, therapeutic options and some epidemiologic data are also discussed.

Clinical history, therapeutic options and some epidemiologic data are also discussed. Os autores apresentam um caso clínico de endocardite bacteriana, envolvendo uma prótese biológica em posição aórtica colocada há cerca de 10 anos, com formação de abcesso perivalvular, num doente de 32 anos de idade. A evolução clínica desta situação foi típica , com progressão de uma síndrome febril arrastada para uma situação de insuficiência ventricular esquerda aguda.
