Este trabalho relata o caso de uma doente de 83 anos de idade, seguida em consulta de Pneumologia desde 2017. Nessa altura iniciou quadro de tosse seca, pieira e dispneia de esforço, que evoluiu progressivamente ao longo dos anos seguintes. Adicionalmente, apresentava, nos exames de imagem, múltiplas áreas de hiperatenuação em vidro despolido e espessamento septal, com padrão em “crazy paving”. Estas alterações parenquimatosas tornaram-se gradualmente mais extensas e difusas, não tendo sido estabelecido previamente um enquadramento etiológico para as mesmas. A doente não comportava comorbilidades relevantes para o quadro clínico atual e não apresentava antecedentes de tabagismo ou historial familiar de relevo. Salientava-se, contudo, historial de exposição inalatória intensa ao fumo dos incêndios florestais de Pedrógão Grande, deflagrados em junho de 2017, referido claramente como tendo precedido o início das manifestações respiratórias. O exame objetivo não evidenciou alterações significantes. Após investigação adicional, concluiu-se, com elevado grau de confiança, pelo diagnóstico de Proteinose Alveolar Pulmonar. Subsequentemente, excluiu-se componente autoimune, bem como as etiologias mais frequentes da forma secundária da patologia. Assim, assumiu-se como mais provável a hipótese de PAP secundária à exposição inalatória massiva a fumo de incêndio florestal. Este é, tanto quanto é do nosso conhecimento, o primeiro relato a propor esta associação.Pulmonary Alveolar Proteinosis (PAP) is a rare parenchymal disease. We present a case report of an 83-year-old female patient who began follow-up with Pneumology in 2017. At the time, she presented with nonproductive cough, wheezing and exertional dyspnea, which gradually became more severe over the following years. Additionally, imaging exams had revealed multiple areas of ground-glass opacities and septal thickening, consistent with the “crazy paving” pattern. These parenchymal findings progressively became more wide-ranging and spread out, eventually affecting all lobes of both lungs. No ethological explanation had been established for these findings and clinical symptoms. The patient’s known comorbidities had no apparent relation with the current symptoms or imaging findings. She was a lifetime non-smoker and no relevant family history was evident. She described a prior episode of massive inhalation exposure to wildfire smoke (Pedrógão Grande Wildfire, June 2017), which clearly appeared to have preceded the onset of symptoms. Physical exam was unremarkable. After further investigation, a diagnosis of Pulmonary Alveolar Proteinosis was established with a high degree of confidence. Subsequently, the autoimmune form of the disease was ruled out, as well as the most common etiologies of secondary PAP. Therefore, we concluded that PAP secondary to massive wildfire smoke inhalation was the most likely explanation for this case.
