Both personality and environmental factors play a part in burnout; and perfectionism, a common trait in medicine students’ personalities, has been linked with increasing students’ vulnerability to burnout. Empathy is essential for good patient care and has been shown to decline with distress and burnout. The literature, thus far, has not explored the construct of burnout and whether this can impact the empathy of perfectionist medical students. Objective: To understand the relationship between perfectionism, burnout and empathy in medical students and to determine whether burnout has a mediating role in the relationship between perfectionism and empathy. Materials and Methods: A sample of 446 medical students from different Universities from Portugal participated in this study, recruited through social media. They answered a Google Forms that included the following validated questionnaires: BIG-3 Perfectionism Scale; Maslach Burnout Inventory-Student Survey (MBI-SS), and the Empathy Scale from Study on Social and Emotional Skills (SSES) from OECD.Results: Empathy showed significant correlation with all variables except with Rigid Perfectionism/RP. Self-Critical perfectionism/SCP showed a significant correlation with all three dimensions of burnout (Academic Efficacy/AE; Emotional Exhaustion/EE; Disengagement). NP demonstrated a positive correlation with total burnout. Mediation analyses revealed that the NP directly predicted lack of empathy, but the mediation effect was not significant. In contrast, the effect of SCP on lack of empathy was fully mediated by all three dimensions of burnout. Discussion and Conclusion: This study highlighted the role of perfectionism in low levels of empathy in medical students. As previously suggested, NP leads to a lack of empathy independently of total burnout levels. SCP alone does not lead to low empathy, however, if the student, due to his/her negative perfectionism, also suffer from burnout, he/she will probably be insufficiently empathetic. This study provides further information on how empathy can be shaped through individual experiences. Therefore, in the future, it would be interesting to analyse if the inclusion of intervention programs based on therapeutic approaches aimed at self-regulation of thoughts and emotions in the medical education curricula would effectively reduce students’ burnout and increase their empathy.Introdução: Estudos recentes têm documentado, entre estudantes de medicina, uma elevada prevalência de burnout do qual advêm efeitos negativos, não apenas a nível pessoal mas também associados ao doente. Tanto a personalidade como fatores ambientais têm um papel importante no burnout, sendo que o perfecionismo, um traço transversal aos estudantes de medicina, tem uma relação com o aumento da vulnerabilidade ao burnout. A empatia é essencial para uma boa prática clinica e tem sido comprovado que tem tendência a diminuir com o sofrimento psicológico e com o burnout. A literatura, até agora, não explorou de que forma o constucto do burnout pode afetar a empatia do estudante de medicina perfeccionista.Objetivo: Compreender a relação entre o perfecionismo, burnout e empatia em estudantes de medicina, analisando o papel do burnout como mediador da relação entre o perfecionismo e a empatia. Materiais e Métodos: Participaram neste estudo 446 estudantes de medicina de diferentes Universidades de Portugal, recrutados através das redes sociais. Responderam a um formulário online Google Forms que incluía os seguintes questionários: BIG-3 Perfectionism Scale, Maslach Burnout Inventory-Student Survey (MBI-SS) e a Escala de Empatia do Study on Social and Emotional Skills (SSES) da OCDE. Resultados: A empatia apresentou uma correlação positiva com todas as variáveis exceto com o perfecionismo rígido. O perfecionismo auto-critico mostrou uma correlação significativa com as três dimensões do burnout (eficácia académia; exaustão emocional; descrença). O perfecionismo narcísico apresentou uma correlação positiva com os níveis de burnout total. As análises de mediação demonstraram que o perfecionismo narcísico teve um efeito direto significativo sobre a diminuição da empatia, mas um efeito mediador indireto não significativo. No entanto, no perfecionismo auto-critico o efeito mediador foi total, pelas três dimensões do burnout, na diminuição da empatia. Discussão e Conclusão: Este estudo sublinha o papel do perfecionismo na diminuição dos níveis de empatia em estudantes de medicina. Como sugerido anteriormente, o perfeconismo narcísico associa-se à diminuição da empatia independentemente dos níveis de burnout. O perfecionismo auto-critico por si só não se associa a reduzidos níveis de empatia, no entanto, se um estudante, através do seu perfecionimo negativo, também sofrer de burnout, provavelmente não será suficientemente empático. Este estudo fornece mais informação de como a empatia pode ser moldada por experiências individuais.
