Amyand’s hernia – Case report and literature review

Resumo: he acute appendicitis’s diagnosis sometimes is a challenge for the General Surgeon. The different positions where appendix may be found and the protean manifestations of acute appendicitis can make the diagnosis possible only intraoperative. The Amyand’s hernia - defined as the appendix included in the hernial sac – is frequently presented as an incarcerated right inguino-scrotal hernia. There is no standard treatment for the Amyand’s hernia. Factors as the presence of an inflamed appendix, the contamination of the operating field, the patient’s age and other anatomical variants must be considered when deciding the correct surgical techniques. The authors report the case of an 82-year-old male observed in the Emergency Department with a voluminous incarcerated right inguino-scrotal hernia. At surgical time it was found an Amyand’s hernia with a gangrenous appendicitis without perforation. An appendicectomy was performed followed by the hernia’s repair according to Rutkow-Robbins technique. The postoperative period was uneventful and the patient was discharged at day 3. A possibilidade do apêndice ileocecal se apresentar em diferentes localizações e a eventual carência de clínica sugestiva podem levar a que o diagnóstico só seja possível intra-operatoriamente. A Hérnia de Amyand, definida como a presença de apêndice ileocecal em saco herniário, apresenta-se muitas vezes como uma hérnia inguino-escrotal direita encarcerada. Não existe um tratamento standard para a resolução da Hérnia de Amyand. Factores como a presença de apêndice inflamado, contaminação do campo operatório, idade do doente e condicionantes anatómicas são determinantes na opção da técnica cirúrgica. Os autores apresentam o caso clínico de um doente do sexo masculino de 82 anos de idade que recorre ao Serviço de Urgência com hérnia inguino-escrotal direita volumosa, com encarceramento. Tratava-se de uma Hérnia de Amyand, tendo sido realizada apendicectomia em apêndice gangrenoso, não perfurado, seguido de correcção do defeito herniário com prótese pela técnica de Rutkow-Robbins. O período de pós-operatório decorreu sem intercorrências relevantes, registando-se alta hospitalar ao 3o dia.
