Amyand’s hernia – Case report and literature review

he acute appendicitis’s diagnosis sometimes is a challenge for the General Surgeon. The different positions where appendix may be found and the protean manifestations of acute appendicitis can make the diagnosis possible only intraoperative. The Amyand’s hernia - defined as the appendix included in the hernial sac – is frequently presented as an incarcerated right inguino-scrotal hernia. There is no standard treatment for the Amyand’s hernia. Factors as the presence of an inflamed appendix, the contamination of the operating field, the patient’s age and other anatomical variants must be considered when deciding the correct surgical techniques. The authors report the case of an 82-year-old male observed in the Emergency Department with a voluminous incarcerated right inguino-scrotal hernia. At surgical time it was found an Amyand’s hernia with a gangrenous appendicitis without perforation. An appendicectomy was performed followed by the hernia’s repair according to Rutkow-Robbins technique. The postoperative period was uneventful and the patient was discharged at day 3.
