Resumo: Psoriasis is a chronic inflammatory disorder, multisystem and immunologically mediated. Approximately one third of adults refer the age of onset before 16, representing about 4% of all dermatoses seen in pediatric age. The majority of cases are mild and adequately managed with topical medications. A minor subset of children present with severe, rapidly evolving disease that requires systemic therapy. The use of these therapies is limited by the risk of toxicity, the low acceptance and lack of studies to validate and guide the use of most of these drugs in children. This is the first of two articles which aim to review the topical (Part I) and systemic (Part II) treatment options available, as well as its safety and efficacy in children.

Cerca de um terço dos adultos acometidos refere início da doença antes dos 16 anos de idade, representando aproximadamente 4% de todas as dermatoses observadas em idade pediátrica. O diagnóstico precoce e a instituição de terapêutica adequada desempenham um papel fundamental na diminuição do impacto físico, emocional e social, bem como das co-morbilidades associadas. Na maioria dos casos a doença é limitada, sendo a terapêutica tópica suficiente para o seu controlo. Contudo, uma minoria das crianças apresenta um quadro grave e rapidamente progressivo que obriga à instituição de medicação sistémica. O recurso a estas terapêuticas, tanto tópicas como sistémicas, é limitado pelos riscos de toxicidade, pela baixa aceitação e ausência de estudos que validem e orientem a utilização da maioria destes fármacos em crianças.
