Resumo: Variant of lichen planus, lichen planus pilaris (follicular) affects the pilosebaceous unit and can cause scarring alopecia. Lichen planus pigmentosum, another form of presentation of the LP, is characterized by papules or hyperchromic macules that can be itching, and is more frequent in melanodermic patients. We present the case of a patient who simultaneously presented two lichen planus subtypes: pilaris on the scalp, neck and forearms and pigmentosum on the face, which is rare. Trichoscopy improved visualization of changes on the scalp. There were hyperchromic macules on the forehead and erythematous follicular papules on the cervical area. After performing biopsies, the diagnosis of lichen planus pilaris on the scalp, lichen planus pigmentosum on the forehead and lichen planus pilaris on the cervical region were all confirmed. Lesions were initially treated with topical steroids, resulting in clinical improvement, but new lesions were presented on the forearms, and so the use of hydroxychloroquine was initiated.

O líquen plano pigmentoso, outra forma de apresentação do líquen plano, caracteriza-se por pápulas ou máculas hipercrômicas que podem ser pruriginosas, sendo mais frequente em pacientes melanodérmicos. Apresentamos o caso de uma paciente que apresentou simultaneamente dois subtipos de líquen plano: pilar no couro cabeludo, região cervical e antebraços e pigmentoso na face, o que é raro. A tricoscopia facilitou a visualização das alterações no couro cabeludo. Na fronte haviam manchas hipercrômicas e na região cervical pápulas foliculares eritematosas. Após a realização de biópsias dos três tipos de lesões, confirmou-se o diagnóstico de líquen plano pilar no couro cabeludo, líquen plano pigmentoso na fronte e líquen plano pilar na região cervical.
