Iron deficiency anemia: detection in the clinical laboratory, using the biochemical ferritin marker

Resumo: The general objective of this study was to provide an analysis from a simple approach to the detection of iron deficiency or iron deficiency anemia, in the clinical laboratory, using the biochemical marker ferritin. To achieve this task, a documentary-bibliographic investigation was proposed, with a methodology framed in content analysis. For the selection of the consulted material, criteria such as original and review studies published in Spanish, contributions and year of publication corresponding to the period 2016-2021 were assumed. However, works from previous years were referenced as they were considered valuable for this research. Among the main conclusions, it was found that: anemia due to iron (Fe) deficiency or iron deficiency has been classified as a global public health problem, since it affects millions of people around the world, both in developed and emerging countries, mainly affecting to children, adolescents and pregnant women. Therefore, one of the objectives of the WHO is to reduce anemia to eradicate all forms of malnutrition. In response to this, in recent years the clinical laboratory has incorporated new biomarkers to those traditionally used, in order to improve their contribution to the diagnosis and monitoring of iron deficiency and, one of the most reliable laboratory methods to determine the lack of Iron in the body is the measurement of the level of ferritin, since its value is usually below the normal range in patients with iron deficiency anemia.

Para el logro de este cometido, se planteó una investigación de carácter documental-bibliográfico, con una metodología enmarcada en el análisis de contenido. Para la selección del material consultado se asumieron criterios como estudios originales y de revisión publicados en español, aportes y año de publicación correspondientes al período 2016- 2021. No obstante, se referenciaron trabajos de años anteriores por considerarse valiosos para esta investigación. Entre las principales conclusiones se encontró que: la anemia por carencia de hierro (Fe) o ferropenia, se ha catalogado como un problema de salud pública global, pues afecta a millones de personas alrededor del mundo, tanto en países desarrollados como emergentes, afectando principalmente a niños, adolescentes y mujeres embarazadas. Por ello, uno de los objetivos de la OMS, es reducir la anemia para erradicar todas las formas de malnutrición. En atención a esto, en los últimos años el laboratorio clínico ha incorporado nuevos biomarcadores a los tradicionalmente empleados, con el fin de mejorar su contribución al diagnóstico y  seguimiento de la ferropenia y, uno de los métodos de laboratorio más fiable para determinar la carencia de hierro en el organismo es la medición del nivel de ferritina, pues,  su valor suele hallarse por debajo del intervalo de normalidad en los pacientes con anemia ferropénica.O objetivo geral deste estudo foi fornecer uma análise a partir de uma abordagem simples para a detecção de deficiência de ferro ou anemia por deficiência de ferro, em laboratório clínico, utilizando o marcador bioquímico ferritina. Para tanto, foi proposta uma investigação bibliográfica documental, com metodologia enquadrada na análise de conteúdo. Para a seleção do material consultado foram considerados critérios como estudos originais e de revisão publicados em espanhol, contribuições e ano de publicação correspondente ao período 2016-2021, porém, trabalhos de anos anteriores foram referenciados por serem considerados valiosos para esta pesquisa . Dentre as principais conclusões, constatou-se que: a anemia por deficiência de ferro (Fe) ou deficiência de ferro tem sido classificada como um problema de saúde pública global, uma vez que atinge milhões de pessoas em todo o mundo, tanto em países desenvolvidos como emergentes, afetando principalmente para crianças, adolescentes e mulheres grávidas. Portanto, um dos objetivos da OMS é reduzir a anemia para erradicar todas as formas de desnutrição.
