Análise histológica e quantitativa dos componentes acinares e estromais das zonas da próstata humana normal

Resumo: | McNeal divided the human prostate into three major anatomical areas: the peripheral zone (PZ), the central zone (CZ), and the transition zone (TZ). Each of these areas is biologically and histologically distinct. The PZ and TZ have clinical significance and are associated with prostate cancer (PC) and benign prostatic hyperplasia (BPH), respectively. To quantitatively and qualitatively analyse the parenchymal and stromal components that constitute the different prostate zones. We assessed 19 samples from each prostate zone. The samples were obtained from necropsies of young people between 18 and 32 years of age with intact urogenital tracts. The samples were fixed in 4% buffered formalin and processed for paraffin embedding. Sections with a thickness of 5 micrometres were obtained from each sample. The sections were stained using histochemical and immunohistochemical techniques to identify the acinar and stromal components of each zone. Photomicrographs were obtained for morphometric analysis using an algorithm based on colour segmentation. Data were analysed using one-way analysis of variance (ANOVA) with the Bonferroni post-test. Differences with p<0.05 were regarded as statistically significant. Collagen fibres were more numerous in the TZ (+40.26%; p=0.0230) than in the PZ. Muscle fibres were also more numerous in the TZ (+47.05%; p=0.0120) than in the PZ. Elastic system fibres in the TZ significantly differed from those in the PZ (+84.61%; p=0.0012) and the CZ (+61.66%; p=0.0074). Similarly, nerves in the PZ (−42.86%; p=0.0107) significantly differed from nerves in the CZ. Epithelial height was lower in the TZ than in the PZ (−30.17%; p=0.0034) and the CZ (−25.01%; p=0.0330). Our objective, quantitative data regarding the various elements that constitute the normal prostate stroma allowed us to reveal differences among prostate zones. This study established patterns for normal parameters and may be used for posterior comparisons in histopathological analysis. |

Cada uma destas zonas seria diferente biológica e histologicamente. A zona periférica e a zona de transição apresentam um significado clínico e estão associadas ao câncer de próstata e a hiperplasia prostática benigna, respectivamente. Caracterizar, qualitativa e quantitativamente, os componentes que constituem as zonas da próstata. Foram analisadas 19 amostras de cada uma das zonas da próstata obtidas de necrópsias de jovens com idade entre 18 e 32 anos e com sistema urogenital íntegro. As amostras foram fixadas em formalina tamponada 4%, e processadas para inclusão em parafina. De cada amostra foram feitos cortes de cinco micrômetros de espessura. Os cortes foram corados por técnicas histoquímicas e imuno-histoquímicas para identificação dos componentes acinares e estromais de cada zona. Foram feitas fotomicrografias para as análises morfométricas realizadas com o software ImageJ utilizando-se uma técnica computacional baseada na segmentação de cores. Os dados foram analisados por one-way ANOVA e pós-teste de Bonferroni, considerando p<0,05 como estatisticamente significativo, utilizando o software GraphPad Prism 6. As fibras colágenas apresentaram-se em maior número na ZT (+40,26%; p=0,0230) quando comparada com a ZP. As fibras musculares também apresentaram-se em maior número na ZT (+47,05%; p=0,0120) quando comparada com a ZP. As fibras do sistema elástico apresentaram diferença estatística na ZT quando comparada com a ZP (+84,61%; p=0,0012) e quando comparada com a ZC (+61,66%; p=0,0074). Os nervos apresentaram diferença estatística na ZC (-42,86%; p=0,0107) quando comparada com a ZP. A altura do epitélio apresentou-se menor na ZT quando comparada com a ZP (-30,17%; p=0,0034) e quando comparada a ZC (-25,01%; p=0,0330). Nossos dados quantitativos sobre os diferentes elementos que constituem o estroma prostático normal nos permitiram mostrar as diferenças entre as zonas prostáticas.
