Resumo: | Our decade-long experience is the stage for the production of this thesis. We present four essays on the clinic and politics in the field of Collective Mental Health in the light of Freud's and Lacan's psychoanalysis, Marx's historical materialism and some subsidies of institutional analysis. Basically, these theoretical-practical references embodied the Psychiatric Reforms around the world which, with exceptions, were experiences that did not shake the structure of the Hospital-centric Medicalizing Psychiatric Paradigm. From this we infer that this dominant paradigm, from the Capitalist Way of Production, also constantly seeks to reform itself in order to maintain its hegemony in the face of social changes. For this reason, we start from the paradigmatic analysis of Mental Health proposed by Costa-Rosa, which delimits four analytical parameters that serve as “guides for the action/reflection” of workers who seek to overcome the Psychiatric Paradigm with the Psychosocial Paradigm in the ethical horizon. Conceiving the Psychiatric Reform as a stage of transformations, the first essay presents the paradigmatic analysis and proposes the need to go beyond reforms with the condition to make use of them. The set of setbacks in public policies in Brazil after the 2016 coup d'état highlights the relevance of Marx's analysis of the State, ideology, class struggle and the dialectical method. The second essay discusses the fact that the Psychosocial Paradigm requires the overcoming of professional specialisms and the training of new Mental Health workers. Locating the transdisciplinary statute of psychoanalysis and historical materialism, it differentiates the disciplinary workers of the Psychiatric Paradigm from the intercessor workers characterized by the ability to suspend the exercise of knowledge-power, so that the subjects of treatment become protagonists. The third essay addresses the four-year experience of implementing reception groups in a Mental Health Clinic, highlighting institutional movements, the clash of paradigmatic views and resistance to this process. Overcoming medicalizing screening and expanding the offer of speech therapies managed to substantially reduce waiting lines and medicalization. The fourth essay, starting from the clinic, deals with innovations in psychoanalysis for listening in the collective devices of reception and treatment, problematizing a form of collectivity that does not disregard the singular and vice versa. It also addresses the increase in deprivations that falls upon the proletarian classes and the potential and limits of psychoanalytic treatment with these subjects. |

Apresentamos quatro ensaios sobre a clínica e a política no campo da Saúde Mental Coletiva à luz da psicanálise de Freud e Lacan, do materialismo histórico de Marx e de alguns subsídios da análise institucional. Basicamente, essas referências teórico-práticas deram corpo às Reformas Psiquiátricas pelo mundo que, salvo exceções, foram experiências que não abalaram a estrutura do Paradigma Psiquiátrico Hospitalocêntrico Medicalizador. Disso inferimos que esse paradigma dominante, derivado do Modo de Produção Capitalista, também busca constantemente reformar-se para manter sua hegemonia em face das mudanças sociais. Por esse motivo, partimos da análise paradigmática da Saúde Mental proposta por Costa-Rosa, que delimita quatro parâmetros analíticos que servem de “guias para a ação/reflexão” dos trabalhadores que buscam suplantar o Paradigma Psiquiátrico tendo o Paradigma Psicossocial no horizonte ético. Concebendo a Reforma Psiquiátrica como um estágio das transformações, o primeiro ensaio apresenta a análise paradigmática e propõe a necessidade de ir além das reformas com a condição de servir-se delas. A conjuntura de retrocessos nas políticas públicas no Brasil pós-golpe de Estado de 2016 evidencia a relevância da teorização de Marx sobre o Estado, a ideologia, a luta de classes e o método dialético. O segundo ensaio discute o fato do Paradigma Psicossocial exigir a superação dos especialismos profissionais e a formação trabalhadores de Saúde Mental de um novo tipo. Situando o estatuto transdisciplinar da psicanálise e do materialismo histórico, diferencia os trabalhadores-disciplinares do Paradigma Psiquiátrico dos trabalhadores-intercessores caracterizados pela capacidade de suspender o exercício do saber-poder, para que os sujeitos do tratamento se tornem protagonistas. O terceiro ensaio aborda a experiência de quatro anos de implantação de grupos de recepção em um Ambulatório de Saúde Mental, destacando os movimentos institucionais, o choque de visões paradigmáticas e as resistências a esse processo. A superação da triagem medicalizadora e a ampliação das ofertas de tratamentos pela fala conseguiram reduzir substancialmente as filas de espera e a medicalização. O quarto ensaio, partindo da clínica, trata das inovações na psicanálise para a escuta nos dispositivos coletivos de recepção e de tratamento, problematizando uma forma de coletividade que não desconsidere o singular e vice-versa.
