Supervised treatment in tuberculosis control: potentialities and fragilities in the nurse’s perception

Resumo: The supervised treatment (TS) is featured as one of the columns of the strategy DOTS (Directly Observed Treatment) it is an important tool to therapeutic adhesion of people with tuberculosis. The research´s objective is to analyze potentialities and fragilities of the ST, in perception of the nurse of the health family program. The qualitative nature research was chosen, done in the sanitary district of João Pessoa – PB in the year 2006. Nine female nurses who worked in health family units (HFU) took part of the study and made the supervised treatment of tuberculosis. Data were collected with semi-structured interview, and to analyzes, the technique of analyzes of content in the modality thematic was used. The fragilities identified were: Lack of incentives for operation of ST, the stigma of the disease, abandon of treatment and difficulty to do the laboratory exams. The potentialities identified were: free medication, decentralization of the ST to the HFU and relation of acceptance and link between the user-professional. The ST is featured as a technique able to enlarge the capacity of interaction and operation of the nurse professional, however it is detached the necessity of ensuring mechanisms that privilege bigger accessibility and integrality of the attention.

La investigación tuvo como objetivo analizar las potencialidades y fragilidades del TS, en la percepción del enfermero del Programa Salud de la Familia. Se optó por la investigación de naturaleza cualitativa, realizada en un distrito sanitario de João Pessoa-PB en 2006. Participaron del estudio nueve enfermeras que trabajan en Unidades de Salud de la Familia (USF) y realizan el tratamiento supervisionado de la tuberculosis. Los datos fueron recolectados por medio de entrevista semiestructurada, y para el análisis, se utilizó la técnica de análisis de contenido, modalidad temática. Las fragilidades identificadas fueron: falta de incentivos para la operacionalización del TS, el estigma de la enfermedad, el abandono del tratamiento y la dificultad en la realización de los exámenes laboratoriales. Las potencialidades observadas fueron: la medicación gratuita, la descentralización del TS para las USF y la relación de acogimiento y vínculo entre usuario-profesional. El TS se caracteriza como una práctica capaz de ampliar la capacidad de interacción y actuación del profesional enfermero, Sin embargo, se destaca la necesidad de asegurar mecanismos que privilegien mayor accesibilidad e integralidad de la atención. O tratamento supervisionado (TS) caracteriza-se como um dos pilares da estratégia DOTS (Directly Observed Treatment), constituindo-se importante ferramenta para a adesão terapêutica dos doentes de tuberculose. Essa pesquisa teve como objetivo analisar as potencialidades e fragilidades do TS, na percepção do enfermeiro do Programa Saúde da Família. Optou-se por um estudo de natureza qualitativa, realizada em um distrito sanitário de João Pessoa-PB no ano de 2006. Participaram do estudo nove enfermeiras que trabalham em Unidades de Saúde da Família (USF) e realizam o tratamento supervisionado da tuberculose. Os dados foram coletados por meio de entrevista semi-estruturada, e para a análise, utilizou-se a técnica de análise de conteúdo, modalidade temática. As fragilidades identificadas foram: falta de incentivos para a operacionalização do TS, o estigma da doença, o abandono ao tratamento e a dificuldade na realização dos exames laboratoriais. As potencialidades observadas foram: a medicação gratuita, a descentralização do TS para as USF e a relação de acolhimento e vínculo entre usuário-profissional.
