Politics of health's regionalization: from the transaction rules to the Pact for Health

Resumo: This is a literature review that aims at identifying the trajectory of the decentralization process and regionalization of the Brazilian Unified Health System, (in Portuguese, SUS) through its structuring legislation; the new municipalities' responsibilities according to the SUS current regionalization policy; and the difficulties and challenges reported by specific literature on the responsibilities of local managers in the process of SUS regionalization. The Scientific Electronic Library Online (Scielo) database was used as well as material published by the Brazilian Health Department. Eight articles on management or regionalization, dating from 2001 to 2009, were found. Thematic content analysis identified three topics addressing decentralization and regionalization. Analyses of articles and current legislation enabled to understand the advances in the consolidation of the SUS as well as the difficulties faced for the construction of a health system that offers services encompassing all levels of care and that guarantee easy access to a comprehensive health care to the population, greater equity and resolvability.

La base de datos empleada para buscar artículos fue la Biblioteca Electrónica Científica en Línea (Scielo) y el material publicado por el Ministerio de Salud de Brasil. Se encontraron ocho artículos que enfocaban estos dos asuntos entre 2001 y 2009. El análisis de contenido permitió identificar tres temas que se referían a descentralización y regionalización. El análisis de los artículos y de la legislación vigente permitió entender los avances en la consolidación del SUS y los obstáculos para construir un sistema de salud que ofrezca servicios en todos los niveles de atención y que garantice atención integral, fácil acceso, mayor equidad y resolución de problemas.Revisão de literatura cujos objetivos são: caracterizar a trajetória do processo de descentralização e regionalização do Sistema Único de Saúde (SUS), por meio da legislação estruturante; conhecer as novas responsabilidades colocadas para os municípios na atual política de regionalização do SUS; e identificar dificuldades e desafios relatados na literatura, em relação às responsabilidades dos gestores municipais no processo de regionalização do SUS. A base de dados utilizada na busca de artigos foi o Scientific Eletronic Library Online (Scielo) e o material publicado pelo Ministério da Saúde do Brasil. Considerando o período de 2001 a 2009, foram encontrados oito artigos em que se abordava gestão ou regionalização. A análise de conteúdo temática permitiu identificar a existência de três temas, definidos a priori, abordando a descentralização e a regionalização.
