Profile of drug-drug interactions and impact on the effectiveness of antiretroviral therapy among patients living with HIV followed at an infectious diseases referral center in Belo Horizonte, Brazil

Resumo: HIV infection may be considered a chronic condition for people living with HIV with access toantiretrovirals and this has effectively increased survival. Moreover, this has also facilitatedthe emergence of other comorbidities increasing the risk for drug–drug interactions andpolypharmacy. The profile of these interactions as well as their consequences for people liv-ing with HIV are still not completely elucidated. The objectives of this study were to describethe profile of these interactions, their prevalence and their classification according to thepotential for significant or non-significant drug–drug interactions. From June 2015 to July2016, people living with HIV on follow-up at an Infectious Diseases Referral Center in BeloHorizonte, Brazil have been investigated for the presence of drug–drug interactions. A total of304 patients were included and the majority (75%) had less than 50 years of age, male (66.4%),and 37.8% self-defined as brown skinned. Approximately 24% were on five or more medica-tions and half of them presented with drug–drug interactions. Patients older than 50 yearshad a higher frequency of antiretrovirals drug–drug interactions with other drugs comparedto younger patients (p = 0.002). No relationship was found between the number of drug–druginteractions and the effectiveness of antiretrovirals. As expected, the higher the number ofnon-HIV medications used (OR = 1.129; 95%CI 1.004–1.209; p = 0.04) was associated with anincrease in drug–drug interactions. The high prevalence of drug–drug interactions foundand the data collected should be useful to establish measures of quaternary prevention andto increase the medication security for people living with HIV.

Além disso, isso também facilitou o surgimento de outras comorbidades, aumentando o risco de interações medicamentosas e polifarmácia. O perfil dessas interações, bem como suas consequências para as pessoas vivendo com HIV, ainda não estão completamente elucidados. Os objetivos deste estudo foram descrever o perfil dessas interações, sua prevalência e sua classificação de acordo com o potencial para interações medicamentosas significativas ou não significativas. De junho de 2015 a julho de 2016, pessoas vivendo com HIV em acompanhamento em um Centro de Referência em Doenças Infecciosas de Belo Horizonte, Brasil, foram investigadas quanto à presença de interações medicamentosas. Foram incluídos 304 pacientes, sendo que a maioria (75%) tinha menos de 50 anos, era do sexo masculino (66,4%) e 37,8% se autodefiniram como pardos. Aproximadamente 24% faziam uso de cinco ou mais medicamentos e metade deles apresentava interações medicamentosas. Pacientes com mais de 50 anos apresentaram maior frequência de interações medicamentosas de antirretrovirais com outras drogas em comparação aos pacientes mais jovens (p = 0,002). Nenhuma relação foi encontrada entre o número de interações medicamentosas e a eficácia dos antirretrovirais. Como esperado, quanto maior o número de medicamentos não anti-HIV usados (OR = 1,129; IC 95% 1,004–1,209; p = 0,04) foi associado a um aumento nas interações medicamentosas.
