Vulnerabilidades e estratégias de enfrentamento de mães de filhos com microcefalia

Resumo: The increase in the prevalence of infants with congenital microcephaly due to infection with the Zika virus has been a challenge not only for scientists, health workers and government officials, but especially for mothers who have experienced a routine of care, , consultations, stimuli, the majority with exclusive dedication. In this sense, based on the Vulnerability Model and the Strategies of Coping, the general objective of this study was to understand elements of vulnerabilities involved in the care of children with microcephaly and to understand the coping strategies used by these mothers. This was an exploratory, analytical study with a qualitative and quantitative approach, using a sociodemographic questionnaire, interview and the Scale of Modes of Confrontation of Problems (EMEP). Participants were 14 mothers of children diagnosed with microcephaly, with a mean age of 24 years. For the analysis of the quantitative data, descriptive statistics were used with the aid of the SPSS, while the analysis of the interviews was done through the technique of thematic categorical analysis. Most of the interviewees were married or lived in a stable relationship, had a family income of up to two minimum wages and had only one child, and the mean age of children who had microcephaly was 1 year and 1 month, where eight of these children were women. Regarding religion, eight women declared themselves to be Catholic, and on the level of religiousness, eleven mothers claimed to be unreligious or religious. Regarding the analysis of the data obtained from the interviews, with respect to the vulnerabilities, the emergence of three thematic classes (from pregnancy to childbirth, care and care of mothers) was observed. categories and subcategories, showing that the history of these women is surrounded by aspects of vulnerability, both individually, socially and programmatically. According to the data analysis of the Scale of Modes of Confrontation of Problems, the type of coping strategy that obtained the highest average was the Confrontation Focused on the Problem (M = 4.30, SD = 0.52), indicating that before the diagnosis of the child's microcephaly, these mothers go deeper into this problem, in order to obtain more knowledge about, seek the best techniques and treatments for each specificity of their children, aiming at the best prognosis for them. However, the other types of Coping Strategies are also used by the participants of this study, which was corroborated by the analysis of the interviews. It has been found that the myth of maternal love permeates these categories, being present at all times in the lives of these women. The results evidenced the wide complexity experienced by these mothers, an arduous context, imbricated with vulnerabilities, which leads to the search for coping strategies, collaborating with the maintenance of mental health.

En este sentido, fundamentado en el Modelo de Vulnerabilidad y en la Teoría de Estrategias de Enfrentamiento, el objetivo general de este estudio fue comprender elementos de vulnerabilidades involucradas en el cuidado de niños con microcefalia y aprehender las estrategias de enfrentamiento utilizadas por estas madres. Se trató de un estudio exploratorio, de carácter analítico, con abordaje cualitativo y cuantitativo, siendo utilizado un cuestionario sociodemográfico, entrevista y la Escala de Modos de Enfrentamiento de Problemas (EMEP). Participaron 14 madres de niños diagnosticados con microcefalia, presentando una media de edad de 24 años. Para el análisis de los datos cuantitativos, se utilizó estadística descriptiva con la ayuda del SPSS, mientras que el análisis de las entrevistas se dio a través de la técnica de análisis categorial temático. La mayoría de las entrevistadas estaban casadas o vivían en una relación estable, tenían ingresos familiares de hasta dos salarios mínimos y tenían sólo un hijo, siendo la media de la edad de los niños que tenían microcefalia de 1 año y 1 mes, donde ocho de estos niños eran del mismo, mujeres. Con respecto a la religión, ocho mujeres se autodeclararon católicas, y sobre el nivel de religiosidad, once madres afirmaron ser poco religiosas o religiosas. En el análisis de los datos obtenidos a partir de las entrevistas, en lo que se refiere a las vulnerabilidades, se observó la emergencia de tres clases temáticas (del embarazo al parto, el cuidado asistencial, la vivencia de las madres acerca del cuidado), las cuales tuvieron sus respectivas las categorías y subcategorías, constatando que la historia de estas mujeres está envuelta por aspectos de vulnerabilidades, tanto en el ámbito individual, como en lo social y programático. De acuerdo con el análisis de los datos de la Escala de Modos de Enfrentamiento de Problemas, el tipo de estrategia de enfrentamiento que obtuvo mayor promedio fue el Enfrentamiento enfocado en el Problema (M = 4,30, DP = 0,52), indicando que ante el, el diagnóstico de microcefalia del hijo, estas madres se profundizan en esta problemática, a fin de obtener más conocimiento sobre, buscar las mejores técnicas y tratamientos para cada especificidad de sus niños, objetivando el mejor pronóstico para ellos. Sin embargo, los otros tipos de Estrategias de Enfrentamiento también son utilizados por las participantes de este estudio, lo que fue corroborado con el análisis de las entrevistas. Se verificó que el mito del amor materno atraviesa por esas categorías, estando presente en todo momento en la vida de esas mujeres. Los resultados evidenciaron la amplia complejidad vivida por estas madres, contexto arduo, imbricado de vulnerabilidades, que lleva a la búsqueda de estrategias de enfrentamiento, colaborando con el mantenimiento de la salud mental.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESO aumento da prevalência de bebês com microcefalia congênita em decorrência da infecção pelo vírus da Zika tem sido um desafio não apenas para os cientistas, trabalhadores da saúde e governantes, mas, principalmente para as mães, as quais têm vivenciado uma rotina de cuidados, exames, consultas, estímulos, a maioria com dedicação exclusiva. Neste sentido, fundamentado no Modelo da Vulnerabilidade e na Teoria de Estratégias de Enfrentamento, o objetivo geral deste estudo foi compreender elementos de vulnerabilidades envolvidas no cuidado de crianças com microcefalia e apreender as estratégias de enfrentamento utilizadas por estas mães. Tratou-se de um estudo exploratório, de caráter analítico, com abordagem qualitativa e quantitativa, sendo utilizado um questionário sociodemográfico, entrevista e a Escala de Modos de Enfrentamento de Problemas (EMEP). Participaram 14 mães de crianças diagnosticadas com microcefalia, apresentando uma média de idade de 24 anos. Para análise dos dados quantitativos, utilizou-se estatística descritiva com o auxílio do SPSS, enquanto que a análise das entrevistas se deu através da técnica de análise categorial temática. A maioria das entrevistadas era casada ou viviam em um relacionamento estável, possuíam renda familiar de até dois salários mínimos e tinham apenas um filho, sendo a média da idade das crianças que tinham microcefalia de 1 ano e 1 mês, onde oito destas crianças eram do sexo feminino. Com relação à religião, oito mulheres se autodeclararam católicas, e sobre o nível de religiosidade, onze mães afirmaram serem pouco religiosas ou religiosas. Acerca da análise dos dados obtidos a partir das entrevistas, no que diz respeito às vulnerabilidades, observou-se a emergência de três classes temáticas (Da gravidez ao parto; O cuidado assistencial; Vivência das mães acerca do cuidado), as quais tiveram suas respectivas categorias e subcategorias, constatando-se que a história destas mulheres é envolta por aspectos de vulnerabilidades, tanto no âmbito individual, quanto no social e programático. De acordo com a análise dos dados da Escala de Modos de Enfrentamento de Problemas, o tipo de estratégia de enfrentamento que obteve maior média foi o Enfrentamento Focado no Problema (M = 4,30; DP = 0,52), indicando que diante do diagnóstico de microcefalia do filho, estas mães se aprofundam nesta problemática, a fim de obter mais conhecimento sobre, buscar as melhores técnicas e tratamentos para cada especificidade das suas crianças, objetivando o melhor prognóstico para eles. Contudo, os outros tipos de Estratégias de Enfrentamento também são utilizados pelas participantes deste estudo, o que foi corroborado com a análise das entrevistas. Verificou-se que o mito do amor materno perpassa por essas categorias, estando presente a todo o momento na vida dessas mulheres.
