O objetivo deste estudo foi avaliar a associação entre EBV e CMV e a presença de AR e sua associação com SS. Foi avaliada uma amostra composta por 256 pacientes de ambos os sexos e maiores que 18 anos, distribuídos em três grupos: artrite reumatoide (AR, n=108), síndrome de Sjögren associada à artrite reumatoide (AR/SS, n=20) e controles saudáveis (C, n=128). Os pacientes foram avaliados quanto à atividade da AR e foram submetidos à sialometria em repouso, teste de Schirmer e biópsia de glândula salivar menor em caso de suspeita de SS. Amostras de sangue foram coletadas para detecção e quantificação das cargas virais de CMV e EBV e amostras de tecido glandular foram utilizadas para detecção destes vírus por hibridização in situ (EBV) e imunohistoquímica (CMV). O EBV foi mais frequente em pacientes com a AR e SS secundária que nos controles saudáveis (p<0,0001), porém sem correlação com os marcadores clínicos (p>0,05) e sem diferença entre os grupos AR e AR/SS (p>0,05). A média do número de cópias do EBV/DNA foi mais elevada em indivíduos portadores de AR (149,42 cópias/μL) e SS (99,24 cópias/μL) que no grupo controle (61,51 cópias/μL, p=0,936). Foi encontrada associação estatisticamente significativa entre o teste de Schirmer e a presença do EBV/DNA (p=0,03). O CMV foi pouco frequente nos dois grupos. Os dois vírus não foram detectados nas biópsias de glândulas salivares menores. A presença do EBV/DNA no sangue periférico porém não sua carga viral está associada à AR, mas não a SS.CNPqThe role of viral infections in the pathogenesis of autoimmune diseases has long been suggested, but available evidence is still available. The objective of this study was to evaluate an association between EBV and CMV and the presence of RA and its association with SS. A sample composed of 256 patients of both sexes and older than 18 years, divided into three groups: rheumatoid arthritis (RA, n=108), Sjögren's syndrome associated with rheumatoid arthritis (RA/SS, n = 20) and healthy controls (C, n = 128). Patients were submitted to resting sialometry, Schirmer's test and minor salivary gland biopsy in case of suspected SS. Blood samples were collected for detection and quantification of viral loads of CMV and EBV and glandular tissue samples for the detection of these viruses by in situ hybridization (EBV) and immunohistochemistry (CMV). EBV was more frequent in patients with RA and secondary SS than in healthy controls (p <0.0001), but no correlation with clinical markers (p>0.05) and no difference between RA and AR/SS (p> 0.05). An EBV/DNA copy number average was higher in RA (149.42 copies/μL) and SS (99.24 copies/μL) non-control (61.51 copies/μL, p=0.936). A statistically significant association was found between the Schirmer test and an EBV/DNA presence (p= 0.03). CMV was uncommon in both groups.
