Defesa : Matinhos, 30/05/2019Inclui referências: p. 87-97Resumo: A preocupação em relação aos impactos na saúde humana causados em decorrência da poluição atmosférica é crescente. Segundo a Organização Mundial de Saúde, estima-se que doenças diretamente relacionadas com a poluição do ar matam cerca de sete milhões de pessoas por ano em todo o mundo, e que na população mundial nove em cada dez pessoas respiram ar poluído e contaminado. Efeitos na morbimortalidade têm sido observados e incluem aumentos em sintomas respiratórios e nos episódios de doença respiratória em crianças. A falta de grandes polos industriais em cidades vizinhas faz de Paranaguá um território ideal para estudos de impactos de poluição do ar advindos da atividade portuária. Neste sentido, o objetivo principal deste estudo consistiu em verificar a correlação entre níveis de poluição atmosférica e a prevalência dos agravos respiratórios de crianças e adolescentes de 0 a 17 anos evidenciados em Paranaguá - PR e correlacionar as áreas de maior atividade portuária com os dados coletados da saúde respiratória da população estudada. Optou-se pela metodologia do estudo epidemiológico observacional analítico do tipo transversal com tendência espaço-temporal. A presente pesquisa teve como amostra informações de prontuários médicos com recorte temporal de 01/01/2012 a 30/09/2017, e relacionados a doenças do aparelho respiratório conforme capítulo X do CID-10, de crianças e adolescentes. Foram incluídas na pesquisa sete UBS e dois centros de referência do município de Paranaguá. Considerando o número de amostras total dos atendimentos por doenças respiratórias, temos 3.005 episódios de doenças respiratórias para o sexo masculino e 2.429 para o sexo feminino. Nota-se uma constante no crescimento do número de atendimentos por doenças respiratórias em relação a idade da criança/adolescente com picos esporádicos aos 4, 7, 11-12 e 13 anos. Para Paranaguá, entre 2012 e 2015, observa-se que os meses que apresentaram os maiores valores foram abril, março, setembro, outubro e novembro. A média de gastos com doenças respiratórias em Paranaguá é de 20,5% por ano em comparação com o valor total gasto com saúde no município, e chegando a custar aos cofres públicos R$1.430.821,65 no período estudado. É possível identificar ainda que, os bairros que apresentaram as maiores prevalências de crises por doenças respiratórias foram o bairro Vila Guadalupe com prevalência de 21,85%, seguido pelos bairros Aeroporto e Colônia Maria Luiza com prevalência de 8,76% e 5,65% respectivamente. Os resultados evidenciam ainda que, temporalmente existe uma correlação positiva para navios, caminhões e gastos do SUS com os atendimentos por doenças respiratórias em Unidades Básicas de saúde e centros de especialidades do Município. Conclui-se então que o impacto da atividade do complexo portuário de Paranaguá, têm influência temporal, espacial, nos gastos de saúde pública, de maior risco, injustiça e vulnerabilidade ambiental, e no processo de adoecimento de sua população. Esta pesquisa corrobora, com a questão do maior risco socioambiental e acometimento de agravos respiratórios em regiões que se sobrepõem, reafirmando a poluição do ar num contexto de injustiça ambiental e em áreas com maior circulação de veículos e menor renda. According to the World Health Organization, air pollution-related illnesses are estimated to kill around seven million people every year around the world, and nine out of ten people in the world breathe polluted and polluted air. Effects on morbimortality have been observed and include increases in respiratory symptoms and episodes of respiratory disease in children. The lack of large industrial poles in neighboring cities makes Paranaguá an ideal territory for studies of air pollution impacts arising from port activity. In this sense, the main objective of this study was to verify the correlation between atmospheric pollution levels and the prevalence of respiratory injuries in children and adolescents from 0 to 17 years of age in Paranaguá - PR and to correlate the areas with greater port activity with the data collected respiratory health of the study population. We chose the methodology of the observational epidemiological study of the transversal type with a spatialtemporal tendency. The present research had as sample information from medical records with a temporal cut from 01/01/2012 to 09/30/2017, and related to respiratory diseases according to chapter X of ICD-10, of children and adolescents. Seven UBS and two reference centers in the city of Paranaguá were included in the study. Considering the total number of samples taken from respiratory diseases, we have 3,005 episodes of respiratory diseases for males and 2,429 for females. There is a constant in the growth in the number of visits for respiratory diseases in relation to the age of the child / adolescent with sporadic peaks at 4, 7, 11-12 and 13 years. For Paranaguá, between 2012 and 2015, it is observed that the months that presented the highest values were April, March, September, October and November. The average cost of respiratory diseases in Paranaguá is 20.5% per year compared to the total amount spent on health in the municipality and costing public coffers R$ 1,430,821.65 in the period studied. It is also possible to identify that the neighborhoods that presented the highest prevalence of crises due to respiratory diseases were the Vila Guadalupe neighborhood with a prevalence of 21.85%, followed by the neighborhoods of Aeroporto e Colônia Maria Luiza, with a prevalence of 8.76% and 5.65 % respectively. The results also show that, temporarily, there is a positive correlation for ships, trucks and SUS expenses with the attendance due to respiratory diseases in Basic Health Units and specialties centers of the Municipality. It is concluded that the impact of the activity of the Paranaguá port complex has a temporal and spatial influence on public health expenditures, greater risk, injustice and environmental vulnerability, and on the disease process of its population. This research corroborates, with the question of greater socioenvironmental risk and involvement of respiratory diseases in overlapping regions, reaffirming air pollution in a context of environmental injustice and in areas with greater circulation of vehicles and lower income.
