Resumo: This study aimed to verify the factors associated with smoking among the older adults and the influence of this habit on systemic arterial hypertension. It is a cross-sectional study carried out with 4023 older adults from 27 municipalities of Minas Gerais. The collection period was between November 2011 and April 2013, using a simple random sample. The data were analyzed by descriptive and inferential statistics, adopting significance when p <0.05. There was a significant association between the older adults smokers and the male (p <0.0001), 60 to 69 years (p <0.0001) and 70 to 79 years (p <0.012); (p <0.0001) and the catholic (p <0.0001) and spiritist religions (p <0.0001); between smoking and systemic arterial hypertension (p <0.0001) and between higher tobacco consumption and the chance of being hypertensive (p <0.031). Smoking is related to males, the first age groups of old age and the catholic and spiritist religions. In addition, smoking directly influenced systemic arterial hypertension.
