Referenciamento e contrareferenciamento à Atenção Primária de à Saúde de idosos com osteoporose

Resumo: | This study aimed to evaluate the referral and counter-referral of elderly people with osteoporosis between a specialized geriatric care center and a specialist in geriatrics and primary health care (PHC). The secondary study developed had two perspectives of analysis: 1) of the patient, made from a coordination with data collection and complementary data collection; 2) Manager of the manager, using telephones, managers of the Basic Health Units (BHU). There were demographic, pharmacotherapeutic, clinical, and service-related variables. A total of 391 patients participated in the study and received their care plan with an indication for the management of osteoporosis. However, 35.3% were not using medication at the time of the interview. Of the total, there is a drop of 31.2% and almost no drop in the last half of life had already shown any extension. For 9.0% of the patients, there was a counter-referral, that is, they had been called to the BHU after attending secondary care. Only 34.3% of patients had the care plan fully implemented. Most patients seen are expected to return to the BHU after receiving the care plan, but just over half agree that its implementation is fully feasible. The main reasons for this are the lack of social/family support or social vulnerability and the lack of professionals in the Family Health Support Centers. Family and health care-related social strategies can be useful to ensure full implementation of care plans for elderly people with osteoporosis. |

Family and health care-related social strategies can be useful to ensure full implementation of care plans for elderly people with osteoporosis.Esse estudo teve como objetivo avaliar o referenciamento e contrarreferenciamento de idosos com osteoporose entre um centro de atenção especializado em geriatria e gerontologia e a atenção primária à saúde. O estudo foi desenvolvido com duas perspectivas de análise: 1)do paciente, feita a partir de uma coorte com coleta de dados secundários e seguimento realizado remotamente; 2)do gestor, utilizando-se inquérito telefônico, direcionada a gestores das Unidades Básicas de Saúde (UBS). Foram descritas variáveis demográficas, farmacoterapêuticas, clínicas e relacionadas ao serviço. Um total de 391 pacientes participaram do estudo e receberam seu plano de cuidados com manejo da osteoporose. Todavia, 35,3% não estavam fazendo uso de medicamento no momento da entrevista. Do total, 31,2% haviam sofrido alguma queda no último ano e quase metade já havia apresentado alguma fratura na vida. Para 89,0% dos pacientes ocorreu o contrarreferenciamento, ou seja, havia sido chamado à UBS após atendimento na atenção secundária. Somente 34,3% dos pacientes tiveram o plano de cuidados totalmente implementado. A maioria dos gestores informou que os pacientes são convidados a retornar à UBS após o recebimento do plano de cuidados, mas pouco mais da metade concorda que sua implementação é totalmente viável. Os principais motivos para isso são a falta de apoio social/familiar ou vulnerabilidade social e falta de profissionais no Núcleo de Apoio à Saúde da Família.
