INVESTIGATION OF THE EPIDEMIC-EPIZOOTIC PROCESS OF YELLOW FEVER IN BRAZILIAN SOUTHEAST REGION IN THE PERIOD 2016-2018

Resumo: Objective: This study aimed to investigate the relationship between the occurrence of yellow fever - YF and the climate and environment during the epidemic-epizootic process in the Brazilian Southeast region from 2016 to 2018. Methods: the methodology considered four approaches: a spatial analysis of the epidemiological situation; detection of clusters of human cases and spatial distribution of epizootics in clusters; identification of environmental and climatic factors; and correlation of the relative risk of the clusters with the associated factors. Results: Most positive cases occurred in Minas Gerais and São Paulo. Clusters were detected in the years 2017 and 2018. The study period had the highest temperature values and the lowest precipitation compared to the 20-year climatological period of analysis. Seasonality marks the temporal pattern of the disease, where most cases occur between December and May. In addition, outbreaks occur with irregular periodicity when the virus finds favorable conditions for transmission. The constant transformation was observed in geographic space, where land use and land cover have the highest percentage linked to rural areas, followed by forest areas. A correlation between the relative risk of clusters and environmental and climatic factors was confirmed. Conclusion: The correlation of climatic and environmental factors and the presence of vectors and non-human primates-PNHs generated a vulnerability scenario for the Southeast region. Public health plans for vaccinating the population and controlling urban vectors should be evaluated to prevent the re-emergence of urban YF.

Métodos: A análise dos dados foi realizada por meio de quatro abordagens: análise espacial da situação epidemiológica; clusters de casos humanos e espacialização das epizootias nos mesmos; identificação de fatores ambientais e climáticos; e a correlação do risco relativo dos clusters e os fatores identificados. Resultados: No período estudado, a maioria dos casos positivos da doença ocorreu no estado de Minas Gerais seguido por São Paulo. Os clusters foram detectados nos anos de 2017 e 2018. O período de estudo apresentou os maiores valores de temperatura e os menores de precipitação em comparação ao período climatológico de 20 anos de análise. A sazonalidade marca o padrão temporal da doença, onde a maioria dos casos ocorre entre dezembro e maio, além disso, surtos ocorrem com periodicidade irregular, quando o vírus encontra condições favoráveis para a transmissão. O espaço se encontra em transformação constante, onde o uso e cobertura do solo possui a maior porcentagem ligada às áreas rurais, seguido das áreas florestais.  Os resultados mostraram uma correlação entre o risco relativo dos clusters como fatores ambientais e climáticos.
