Investigações mais detalhadas sobre o impacto da obesidade e do emagrecimento rápido nos vários domínios da vida social são escassas. O objetivo deste estudo foi analisar (a) os reforçadores disponíveis e acessíveis ao portador de obesidade grau III antes e após tratamento cirúrgico, segundo sua natureza; (b) as variáveis que definem a variação na disponibilidade e acesso aos reforçadores no pós-operatório; (c) o custo da resposta de acesso aos reforçadores antes e depois da operação. Participaram do estudo 30 pacientes do Ambulatório de Obesidade do Hospital Universitário de Brasília, sendo 15 em lista de espera (9F; 6M) e 15 operados (10F; 5M). Os participantes em lista de espera tinham IMC entre 40,84 kg/m2 e 67,16 kg/m2 (X = 51,31 kg/m2), idade entre 21 e 64 anos e escolaridade predominantemente primária. Os participantes operados tinham 6,1 a 31 meses de operados, IMC pré-operatório entre 39,82 kg/m2 e 53,12 kg/m2 (X = 46,52 kg/m2) e IMC pós-operatório entre 24,9 kg/m2 e 39,1 kg/m2 (X = 30,44 kg/m2), idade entre 26 e 53 anos e escolaridade predominantemente secundária. Foi realizada uma entrevista semi-estruturada individual e aferido o peso e altura de cada participante. Os resultados mostraram 15 classes de reforçadores – aceitação social, afeto, elogio, rede de apoio, vida social, amizade, lazer, atividade física, flerte, vestuário, sexo, relacionamentos amorosos, estudo, trabalho, e transporte público – com acesso médio de 59,2% para participantes não-operados e de 83,1% para operados. Foram identificadas variáveis relacionadas à obesidade, ao tratamento e de outra natureza que funcionavam como barreiras ao acesso ou como estímulos aversivos que oneravam o custo da resposta tornando conflituoso o acesso aos reforçadores. As variáveis relacionadas à obesidade foram relatadas de modo prevalente entre não-operados e agrupadas em variáveis ambientais, orgânicas, pessoais e sociais. As variáveis do tratamento se caracterizavam por impor uma limitação temporária aos participantes operados. Os resultados sugerem uma melhora considerável na freqüência e qualidade de acesso aos reforçadores após a perda ponderal e a importância da assistência psicológica no tratamento pré e pós-operatório do grande obeso. Estudos sobre a manutenção desse acesso a longo prazo e a comparação entre obesos operados e pessoas de peso normal são sugestões para estudos futuros. ________________________________________________________________________________ ABSTRACTResearch data show a significant improvement in physical health and little or no improvement in mental health among obese people who undergo bariatric surgery. Further investigations on the effects of obesity and weight loss on one’s social life are still necessary. This study was aimed to investigate the (a) reinforcement availability and accessibility by obese people in their natural environment before and after bariatric surgery; (b) predictors of variation in the reinforcement availability and accessibility after the operation; (c) cost of response to access the reinforcement before and after the operation. Participants were 30 outpatients attending the Obesity Ambulatory of the Hospital Universitário de Brasília. From those, 15 patients came from the waiting list (9F;6M), had BMI between 40,84kg\m2 and 67,16kg/m2 (X=51,31kg/m2), age 21 to 64-years-old, and most of them had elementary school education. The other 15 had already undergone the gastroplasty (10F;5M), had pre-surgery BMI varying between 39,82kg/m2 and 53,12/m2 (X=46,52kg/m2) and post-surgery BMI between 24,9kg/m2 and 39,1kg/m2 (X=30,44kg/m2), age 26 to 53-years-old, and most of them had high-school education. Data were gathered by an individual semi-structured interview after weight and height measurement. Results showed 15 classes of reinforcement – social acceptance, affection, praise, social support network, friendship, leisure, physical activity, flirting, loving relationships, sexual life, social life, study, work, clothing and public transportation – with an average access of 59,2% by not-yet-operated patients and 83,1% by operated patients. It was found some variables related to obesity, treatment and else more, which worked as barriers to reinforcement access or as aversive stimuli. The nature of the variables related to obesity was environmental, organic, personal and social. Those variables were reported more often by the not-yet-operated participants. The variables related to treatment were those implying some temporary limitations to operated participants. Overall, results showed a higer increase in the frequency and quality of reinforcement access by participants after losing weight and the necessity for psychological assistance for obese patients before the surgical treatment.
