Obstacles in the implementation of evidence-based practice (EBP) in nursing: an integrative review

Resumo: Florence Nightingale, a pioneer in executing evidence-based practice (PBE), which is an approach inserted in the Health Sciences that associates the best scientific evidence with clinical experience and the patient's choice for effective clinical decision making, enabling the higher education and quality nursing practice. The objective is to investigate what are the main obstacles in the implementation of evidence-based practice - EBP in nursing. It is an integrative literature review. Data collection was carried out from April to July 2020, in the PubMed (US National Library of Medicine) and SciELO (Scientific Electronic Library Online) databases. For the search strategy, Health Science Descriptors (DeCS) and Medical Subject Headings (MeSH) were used. Using the Boolean operators “and / or”, in the different crossings of the pre-defined languages. Manual counting, that is, absolute frequency of similar terms, was used as a strategy for the synthesis of the studies, organizing the results in categories. Seven primary studies that met the inclusion criteria, and made up this integrative review. As for the temporal distribution of research, it is observed that there is no linear trend pattern, with a greater concentration of 57.14% in 2018. Regarding the methodological design of the sample, qualitative studies stood out 35.71%. The scientific basis of PBE, ensures that care is changeable and dynamic. Although nurses have attitudes favorable to EBP and believe that research contributes to the advancement of Nursing, this practice is still limited due to the obstacles: work overload; nurses do not want to change the care practice and do not understand the design of the EBP. More support and resources from higher education institutions in nursing at undergraduate and graduate levels and therapeutic units are needed, so that students and nursing professionals can perform EBP on a daily basis. The results presented here can support the development of pilot studies for the implementation of Evidence-Based Practice among nurses.

Florence Nightingale, pionera en la ejecución de la práctica basada en la evidencia (PBE), que es un enfoque insertado en las Ciencias de la Salud que asocia la mejor evidencia científica con la experiencia clínica y la elección del paciente para la toma de decisiones clínicas efectivas, permitiendo la educación superior y práctica de enfermería de calidad. El objetivo es investigar cuáles son los principales obstáculos en la implementación de la práctica basada en la evidencia - PBE en enfermería. Es una revisión integradora de la literatura. La recolección de datos se realizó de abril a julio de 2020, en las bases de datos PubMed (Biblioteca Nacional de Medicina de EE. UU.) Y SciELO (Biblioteca Electrónica Científica en Línea). Para la estrategia de búsqueda, se utilizaron descriptores de ciencias de la salud (DeCS) y Medical Subject Headings (MeSH). Utilizando los operadores booleanos “y / o”, en los diferentes cruces de los lenguajes predefinidos. El conteo manual, es decir, la frecuencia absoluta de términos similares, se utilizó como estrategia para la síntesis de los estudios, organizando los resultados en categorías. Siete estudios primarios que cumplieron con los criterios de inclusión e integraron esta revisión integradora. En cuanto a la distribución temporal de la investigación, se observa que no existe un patrón de tendencia lineal, con una mayor concentración del 57,14% en 2018. En cuanto al diseño metodológico de la muestra, se destacaron los estudios cualitativos 35,71% . La base científica de PBE asegura que la atención sea cambiante y dinámica. Si bien las enfermeras tienen actitudes favorables a la PBE y creen que la investigación contribuye al avance de la Enfermería, esta práctica aún está limitada por los obstáculos: sobrecarga de trabajo; las enfermeras no quieren cambiar la práctica asistencial y no comprenden el diseño de la PBE. Se necesitan más apoyos y recursos de las instituciones de educación superior en enfermería a nivel de pregrado y posgrado y unidades terapéuticas, para que los estudiantes y profesionales de enfermería puedan realizar la PBE a diario. Objetiva-se investigar quais são os principais entraves na implementação da prática baseada em evidências – PBE em enfermagem. Trata-se de uma revisão integrativa da literatura. A coleta de dados foi realizada de abril a julho de 2020, nas bases de dados PubMed (US National Library of Medicine) e SciELO (Scientific Electronic Library Online). Para a estratégia de busca, utilizou-se dos Descritores da Ciência da Saúde (DeCS) e dos Medical Subject Headings (MeSH). Utilizando-se dos operadores booleanos “and/ or”, nos diferentes cruzamentos dos idiomas pré-definidos. Utilizou-se como estratégia para a síntese dos estudos a contagem manual, isto é, frequência absoluta dos termos similares, organizando os resultados em categorias. Sete estudos primários que atenderam aos critérios de inclusão, e compuseram esta revisão integrativa. Quanto à distribuição temporal das pesquisas, observa-se que não há um padrão tendencial linear, com concentração maior 57,14% no ano de 2018. Em relação ao desenho metodológico da amostra, sobressaiu-se os estudos do tipo qualitativo 35,71%. A base científica da PBE, garante que o cuidado seja mutável e dinâmico.  Apesar de os enfermeiros apresentarem atitudes favoráveis à PBE e acreditarem que as pesquisas contribuem para o avanço da Enfermagem, essa prática ainda se encontra limitada devido aos entraves: sobrecarga de trabalho; enfermeiros não desejam mudar a prática assistencial e não compreendem o delineamento da PBE. Torna-se necessário mais apoio e recurso por parte das instituições de ensino superior de enfermagem nos níveis de graduação e pós-graduação e unidades terapêuticas, para que assim os estudantes e profissionais de enfermagem, executem a PBE diariamente.
