Síndrome da fragilidade como preditora de desfechos adversos em idosos hospitalizados: uma revisão sistemática e metanálise

Summary: INTRODUÇÃO: A síndrome da fragilidade é prevalente em idosos hospitalizados, assim como a ocorrência de desfechos adversos. OBJETIVO: Verificar por meio de uma revisão sistemática se a síndrome da fragilidade em idosos hospitalizados prediz desfechos adversos. MÉTODOS: Realizou-se busca manual e eletrônica (PUBMED, EMBASE, Web of Science, Lilacs, CINAHL, PsycINFO e Google Scholar), sem limitação de idioma e com data de publicação a partir de 2001. Foram incluídos estudos observacionais prospectivos com idosos (≥ 60 anos) hospitalizados avaliados quanto à síndrome da fragilidade na admissão hospitalar. Os desfechos primários foram declínio funcional na hospitalização e mortalidade após alta hospitalar. Os demais dados avaliados foram considerados desfechos secundários. A qualidade metodológica dos estudos foi avaliada pelo Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies. Foi realizada análise crítica do conteúdo dos estudos e metanálise. RESULTADO: Foram incluídos 23 artigos, correspondentes a 15 estudos. A fragilidade foi avaliada por 11 instrumentos. O declínio funcional foi avaliado por quatro estudos, sendo que ser não-frágil é um fator de proteção para tal desfecho comparado em ser frágil [RR: 0,76 (95% IC: 0,60-0,97)]. A mortalidade após a hospitalização foi avaliada por 13 estudos, sendo menor o risco a médio e longo prazo tanto para idosos não-frágeis [médio-RR: 0,11 (95% IC: 0,02-0,52) e longo- RR: 0.13 (95% IC: 0,08-0,21)] como pré-frágeis [médio-RR: 0,30 (95% IC: 0,09-0,96) e longo-[RR: 0,27 (95% IC: 0,08-0,86)] quando comparado aos idosos frágeis. A fragilidade também é preditora da mortalidade intra-hospitalar e de maior tempo de internação. A qualidade metodológica dos estudos foi considerada de moderada a boa. CONCLUSÃO: A fragilidade em idosos hospitalizados prediz risco para o declínio funcional na hospitalização, maior tempo de internação, e maior mortalidade intra- hospitalar, a médio e a longo prazo.

METHODS: We conducted manual and electronic searches (PUBMED, EMBASE, Web of Science, Lilacs, CINAHL, PsycINFO and Google Scholar), with no language restriction and published since 2001. Prospective observational studies with hospitalized older people (≥ 60 years) evaluated for frailty syndrome at hospital admission were included. Primary outcomes were functional decline at hospital discharge and mortality after discharge. Other data were considered secondary outcomes. The methodological quality of the studies was evaluated by the Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies. A critical analysis and meta-analysis were performed. RESULTS: 23 articles were included, corresponding to 15 studies. Frailty was evaluated by 11 instruments. Functional decline was assessed by four studies, showing that being non-frail is a protective factor for such outcome compared to being frail [RR: 0.76 (95% CI: 0.60-0.97)]. Mortality after hospitalization was evaluated in 13 studies, with the medium- and long-term risk being lower for both non-frail (mean RR: 0.11 (95% CI: 0.02-0.52) and long- RR: 0.13 (95% CI: 0.08-0.21)] and pre-frail [RR: 0.30 (95% CI: 0.09-0.96) and long-RR: 0, 27 (95% CI: 0.08-0.86)] comparing to frail older people. Frailty is also a predictor of in-hospital mortality and longer hospital stay. The methodological quality of the studies was considered moderate to good. description INTRODUÇÃO: A síndrome da fragilidade é prevalente em idosos hospitalizados, assim como a ocorrência de desfechos adversos. OBJETIVO: Verificar por meio de uma revisão sistemática se a síndrome da fragilidade em idosos hospitalizados prediz desfechos adversos. MÉTODOS: Realizou-se busca manual e eletrônica (PUBMED, EMBASE, Web of Science, Lilacs, CINAHL, PsycINFO e Google Scholar), sem limitação de idioma e com data de publicação a partir de 2001. Foram incluídos estudos observacionais prospectivos com idosos (≥ 60 anos) hospitalizados avaliados quanto à síndrome da fragilidade na admissão hospitalar. Os desfechos primários foram declínio funcional na hospitalização e mortalidade após alta hospitalar. Os demais dados avaliados foram considerados desfechos secundários. A qualidade metodológica dos estudos foi avaliada pelo Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies. Foi realizada análise crítica do conteúdo dos estudos e metanálise. RESULTADO: Foram incluídos 23 artigos, correspondentes a 15 estudos. A fragilidade foi avaliada por 11 instrumentos. O declínio funcional foi avaliado por quatro estudos, sendo que ser não-frágil é um fator de proteção para tal desfecho comparado em ser frágil [RR: 0,76 (95% IC: 0,60-0,97)]. A mortalidade após a hospitalização foi avaliada por 13 estudos, sendo menor o risco a médio e longo prazo tanto para idosos não-frágeis [médio-RR: 0,11 (95% IC: 0,02-0,52) e longo- RR: 0.13 (95% IC: 0,08-0,21)] como pré-frágeis [médio-RR: 0,30 (95% IC: 0,09-0,96) e longo-[RR: 0,27 (95% IC: 0,08-0,86)] quando comparado aos idosos frágeis. A fragilidade também é preditora da mortalidade intra-hospitalar e de maior tempo de internação. A qualidade metodológica dos estudos foi considerada de moderada a boa. CONCLUSÃO: A fragilidade em idosos hospitalizados prediz risco para o declínio funcional na hospitalização, maior tempo de internação, e maior mortalidade intra- hospitalar, a médio e a longo prazo.
