Congenital Toxoplasmosis Infection: case report

Resumo: | This study aimed to report the case of a 26-year-old woman, brown, single, homeless and drug user, with a gestational age of 35 weeks, who did not undergo prenatal care or obtained any medical assistance during pregnancy, attended in the emergency department of a maternal and child reference institution in Belém in labor, where she was treated and underwent surgical delivery, giving birth to fraternal twins. Due to the clinical profile of those born, an investigation for Congenital Toxoplasmosis was carried out while still in the hospital. In both cases, seropositivity of anti-Toxoplasma gondii (T. gondii) IgM and IgG antibodies was detected in the first days of life. After diagnosis, specific therapy was instituted and both were referred to the referral center for infection in Belém, where they began follow-up at 2 months of age. The twins followed a routine investigation for the disease, which included fundoscopy, performed in the first months, computed tomography of the skull at 7 months and electroencephalogram at 1 year of life. This report shows a rare situation of Congenital Toxoplasmosis affecting two dizygotic twins. Factors such as maternal condition, virulence of the strain involved and/or the parasitic inoculum may have contributed to the systemic involvement of both to different degrees. Prevention measures at all levels are of paramount importance to reduce the risk of cases like this, as well as adequate support after birth, to minimize the sequelae of infection. |

Prevention measures at all levels are of paramount importance to reduce the risk of cases like this, as well as adequate support after birth, to minimize the sequelae of infection.Este estudio tuvo como objetivo relatar el caso de una mujer de 26 años, morena, soltera, indigente y consumidora de drogas, con edad gestacional de 35 semanas, que no realizó control prenatal ni obtuvo asistencia médica durante el embarazo, atendida en el emergencia de una institución de referencia materno-infantil de Belém en trabajo de parto, donde fue atendida y se le realizó parto quirúrgico, dando a luz a mellizos. Debido al cuadro clínico de los nacidos, se realizó investigación por Toxoplasmosis Congénita estando aún en el hospital. En ambos se detectó seropositividad de anticuerpos IgM e IgG anti-Toxoplasma gondii (T. gondii) en los primeros días de vida. Posterior al diagnóstico se instauró terapia específica, y ambas fueron derivadas al centro de referencia de infecciones de Belém, donde iniciaron seguimiento a los 2 meses de edad. Los gemelos siguieron una investigación de rutina para la enfermedad, que incluyó fundoscopía, realizada en los primeros meses, tomografía computarizada de cráneo a los 7 meses y electroencefalograma al año de vida. Este informe muestra una situación rara de Toxoplasmosis Congénita que involucra a dos gemelos dicigóticos. Factores como la condición materna, la virulencia de la cepa involucrada y/o el inóculo parasitario pueden haber contribuido al compromiso sistémico de ambos en diferentes grados. Las medidas de prevención a todos los niveles son de suma importancia para reducir el riesgo de casos como este, así como el apoyo adecuado después del nacimiento, para minimizar las secuelas de la infección.Este estudo teve como objetivo relatar o caso de uma mulher de 26 anos, parda, solteira, moradora de rua e usuária de drogas, com idade gestacional de 35 semanas, não realizou o pré-natal ou obteve qualquer assistência médica durante a gravidez, atendida no serviço de urgência de instituição de referência materno-infantil de Belém em trabalho de parto, onde foi atendida e submetida a parto cirúrgico, dando à luz a gêmeas bivitelinas. Devido ao perfil clínico das nascidas, foi realizada investigação para Toxoplasmose Congênita ainda no hospital. Em ambas se detectou soropositividade dos anticorpos IgM e IgG anti-Toxoplasma gondii (T. gondii) nos primeiros dias de vida. Após o diagnóstico, foi instituída a terapia específica e ambas foram encaminhadas ao centro de referência para a infecção em Belém, onde iniciaram acompanhamento com 2 meses de idade. As gêmeas seguiram rotina de investigação para a doença, que incluiu fundoscopia, realizada ainda nos primeiros meses, tomografia computadorizada de crânio aos 7 meses e eletroencefalograma com 1 ano de vida. Este relato mostra uma situação rara de Toxoplasmose Congênita acometendo duas gemelares dizigóticas. Fatores como a condição materna, virulência da cepa envolvida e/ou o inóculo parasitário podem ter contribuído para o comprometimento sistêmico de ambas em diferentes graus.
