Mandibular third molar coronectomy as an alternative for inferior alveolar nerve prevention

Resumo: The surgery of impacted third molars has become frequent in the routine of oral and maxillofacial professionals. Coronectomy or partial intentional odontectomy is a surgical procedure in which the dental crown is removed and the roots belonging to this element are left in the dental alveolus. This method aims to preserve the inferior alveolar neurovascular bundle, which exists in the mandibular canal, in cases where the dental root is in close contact with it. The surgical technique of intentional partial odontectomy consists in removing the coronal portion of the tooth 1 to 2 mm below the cemento-enamel junction. Two important points of the technique is that no enamel remains in the dental fragment that will be buried and that it is retained at least 3 mm apical to the alveolar bone crest. As it is a specific surgical technique, the indications for partial odontectomy are restricted to the prevention of lesions to the inferior alveolar nerve, in which the root of the third molar is in close contact with the nerve. This study performed a literature review on the technique of intentional partial odontectomy, its history, technique, indications, contraindications, complementary exams and complications. Partial intentional odontectomy is a viable alternative technique with the purpose of preventing damage to the nerve structures during the exodontia of unerupted mandibular third molars. One of the complications of the procedure is the migration of the remaining roots, but reintervention to remove them can be considered part of the treatment.

La coronectomía u odontectomía parcial intencionada es un procedimiento quirúrgico en el que se elimina la corona dental y se dejan las raíces pertenecientes a este elemento en el alvéolo dental. Este método pretende preservar el haz neurovascular alveolar inferior, que existe en el canal mandibular, en los casos en que la raíz dental está en estrecho contacto con él. La técnica quirúrgica de la odontectomía parcial intencionada consiste en eliminar la porción coronal del diente de 1 a 2 mm por debajo de la unión cemento-esmalte. Dos puntos importantes de la técnica son que no quede esmalte en el fragmento dental que será enterrado y que se retenga al menos 3 mm apicalmente a la cresta ósea alveolar. Al tratarse de una técnica quirúrgica específica, las indicaciones de la odontectomía parcial se limitan a la prevención de lesiones del nervio alveolar inferior, donde la raíz del tercer molar está en estrecho contacto con el nervio. Este estudio realizó una revisión bibliográfica sobre la técnica de la odontectomía parcial intencional, sobre la historia, la técnica, las indicaciones, las contraindicaciones, los exámenes complementarios y las complicaciones. La odontectomía parcial intencionada es una técnica alternativa viable con el fin de evitar el daño de las estructuras nerviosas en las exodoncias de las inclusiones de terceros molares inferiores. Una de las complicaciones del procedimiento es la migración de las raíces restantes, pero la reintervención para eliminarlas puede considerarse parte del tratamiento.A cirurgia de terceiros molares impactados tem se tornado frequente na rotina dos profissionais bucomaxilofaciais. Esse método visa a preservação do feixe vásculo-nervoso alveolar inferior, existente no canal mandibular, em casos que a raiz dentária se apresenta em íntimo contato com o mesmo. A técnica cirúrgica de odontectomia parcial intencional consiste na remoção da porção da coronária do elemento dentário 1 a 2 mm abaixo da junção cemento-esmalte. Dois pontos importantes da técnica é não deixar remanescente de esmalte no fragmento dentário que será sepultado e que este fique retido no mínimo 3mm apicalmente à crista óssea alveolar. Como se trata de uma técnica cirúrgica específica, as indicações para odontectomia parcial se restringem à prevenção de lesões ao nervo alveolar inferior, em que raiz do terceiro molar se encontra em íntimo contato com o nervo. O presente estudo realizou uma revisão de literatura sobre a técnica de odontectomia parcial intencional, sobre histórico, técnica, indicações, contraindicações, exames complementares e complicações. A odontectomia parcial intencional é uma técnica alternativa viável com o intuito de prevenção de danos às estruturas nervosas nas exodontias de terceiros molares inferiores inclusos.
