Hysterocele gravidarum associated with diaphragmatic hernia in a female dog

Resumo: The objective of this study is to report the case of a female dog with hysterocele gravidarum associated with driaphragmatic hernia. The animal presented clinical signs of dyspnea, tachypnea, muffled cardiac auscultation, presence of greenish secretion in the vulva and moderate pain on abdominal palpation. An ultrasound examination was performed based on the clinical suspicion of dystocic delivery, and the presence of two viable fetuses was confirmed. However, given the animal's history, worsening and severity of the clinical condition, the animal was immediately referred for a surgical procedure for cesarean section. After accessing the abdominal cavity and attempting to retract the uterine horns, exposure was difficult, and it was necessary to increase the incision in a cranial direction. Thus, the presence of diaphragmatic rupture with part of the intestinal loops, right kidney and gravid uterus in the thoracic cavity was evidenced. During the procedure, the patient had a cardiorespiratory arrest and died. Thus, necropsy was chosen, which revealed diaphragmatic hernia, dystocic delivery and pulmonary atelectasis with consequent respiratory failure. It is concluded that the surgical correction of diaphragmatic hernias on an emergency basis is of paramount importance and, in cases where there is concomitant pregnancy, the severity of the effects makes the prognosis bad.

El animal presentó signos clínicos de disnea, taquipnea, auscultación cardiaca amortiguada, presencia de secreción verdosa en la vulva y dolor moderado a la palpación abdominal. Ante la sospecha clínica de parto distócico se realizó ecografía y se confirmó la presencia de dos fetos viables. Sin embargo, dados los antecedentes del animal, el empeoramiento y la gravedad del cuadro clínico, el animal fue remitido de inmediato para un procedimiento quirúrgico de cesárea. Tras acceder a la cavidad abdominal e intentar retraer los cuernos uterinos, hubo dificultad para exponerlos, siendo necesario aumentar la incisión en sentido craneal. Así, se evidenció la presencia de ruptura diafragmática con parte de asas intestinales, riñón derecho y útero grávido en cavidad torácica. Durante el procedimiento, el paciente tuvo un paro cardiorrespiratorio y falleció. Por lo tanto, se optó por la necropsia, que reveló hernia diafragmática, parto distócico y atelectasia pulmonar con consecuente insuficiencia respiratoria. Se concluye que la corrección quirúrgica de las hernias diafragmáticas de forma urgente es de suma importancia y, en los casos en que existe un embarazo concomitante, la gravedad de los efectos hace que el pronóstico sea desfavorable.O objetivo deste trabalho é relatar o caso de uma cadela com histerocele gravídica associada a hérnia driafragmática. O animal apresentava sinais clínicos de dispneia, taquipneia, ausculta cardíaca abafada, presença de secreção de coloração esverdeada na vulva e algia moderada à palpação abdominal. Foi realizado exame ultrassonográfico baseado na suspeita clínica de parto distócico, sendo constatada a presença de dois fetos viáveis. Porém dado o histórico do animal, piora e gravidade do quadro clínico, o animal foi encaminhado imediatamente para realização de procedimento cirúrgico de cesárea. Após acesso à cavidade abdominal e tentativa de retração dos cornos uterinos, houve dificuldade para exposição, sendo necessário realizar o aumento da incisão em sentido cranial. Com isso, foi evidenciada a presença de ruptura diafragmática com parte das alças intestinais, rim direito e útero gravídico na cavidade torácica. Durante o procedimento a paciente apresentou uma parada cardiorrespiratória e veio a óbito. Assim, optou-se pela necropsia, que revelou hérnia diafragmática, parto distócico e atelectasia pulmonar com consequente insuficiência respiratória.
