Conservative treatment of intact ectopic pregnancy with methotrexate

Resumo: Introduction: Ectopic pregnancy (EP) is generally defined as the implantation of the blastocyst outside the uterine cavity, synonymous with extrauterine pregnancy. Objective: To analyze the effectiveness of conservative treatment with methotrexate (MTX) in intact EP. Methods: This is a systematic review carried out through extensive research in the medical literature using the electronic databases: Medline (PubMed) and LILACS, with the following search strategy: ("Pregnancy, Ectopic"[Majr] AND "Drug Therapy "[Mesh]). Results: The selected studies converge to demonstrate the effectiveness of conservative treatment with MTX in intact EP and were grouped into three main groups: Patients undergoing conservative treatment with MTX in the single-dose (SD) protocol, patients undergoing the double-dose (DD) protocol and patients undergoing conservative treatment who do not meet the protocol criteria for the use of MTX. The results showed that the MTX SD protocol has sufficient efficacy in the treatment of EP, but in patients outside the parameters required by the current protocol, treatment success increases with the use of the DD or multiple dose method. Conclusions: The SD protocol with the option to elaborate for a second dose in case of treatment failure may be the ideal treatment for intact EP.

Objetivo: Analizar la efectividad del tratamiento conservador con metotrexato (MTX) en el EE intacto. Métodos: Se trata de una revisión sistemática realizada a través de una extensa investigación en la literatura médica utilizando las bases de datos electrónicas: Medline (PubMed) y LILACS, con la siguiente estrategia de búsqueda: ("Pregnancy, Ectopic"[Majr] AND "Drug Therapy" [Mesh ]). Resultados: Los estudios seleccionados convergen para demostrar la eficacia del tratamiento conservador con MTX en el EE intacto y se agruparon en tres grupos principales: Pacientes en tratamiento conservador con MTX en protocolo de dosis única (DU), pacientes en protocolo de doble dosis (DD) y pacientes en tratamiento conservador que no cumplen los criterios del protocolo para el uso de MTX. Los resultados mostraron que el protocolo MTX DU tiene suficiente eficacia en el tratamiento de la EE, pero en pacientes fuera de los parámetros requeridos por el protocolo actual, el éxito del tratamiento aumenta con el uso del método DD o dosis múltiple. Conclusiones: El protocolo de DU con la opción de elaborar una segunda dosis en caso de fracaso del tratamiento puede ser el tratamiento ideal para el EE intacto.Introdução: A gravidez ectópica (GE) é definida, de forma geral, como a implantação do blastocisto fora da cavidade uterina, sinônimo de gravidez extrauterina. Objetivo: Analisar a eficácia do tratamento conservador com metotrexato (MTX) na GE íntegra. Métodos: Trata-se de revisão sistemática realizada através de ampla pesquisa na literatura médica utilizando as bases de dados eletrônicas: Medline (PubMed) e LILACS, com a seguinte estratégia de busca: ("Pregnancy, Ectopic"[Majr] AND "Drug Therapy"[Mesh]). Resultados: Os estudos selecionados convergem para a demonstração da eficácia do tratamento conservador com MTX na GE íntegra e foram agrupados em três grupos principais: Pacientes submetidas ao tratamento conservador com MTX no protocolo de dose única (DU), pacientes submetidas ao protocolo de dose dupla (DD) e pacientes submetidas ao tratamento conservador e que não atendem os critérios do protocolo para o uso do MTX.
