Problems and strategies related to patient safety in primary health care

Resumo: Objectives: To describe the main problems related to patient safety in Primary Health Care from the perspective of nurses; list change strategies that strengthen patient safety in Primary Health Care. Method: Descriptive research, with a quantitative and qualitative approach. In the quantitative one, the Medical Office Survey on Patient Safety Culture instrument was used and in the qualitative one, the causal chain model and the Organizational Development Theory were chosen. Data collection took place from July to December 2017, in 14 health units in a city in southern Brazil. 51 nurses participated. Results: Patient safety problems related to: equipment, exams, medical records, professional quantity, haste, work flow, service management, quantity being more important than quality, information exchange and to professionals who believe that mistakes can be used against you. In view of these, generic strategies were developed, related to the structure, such as equipment maintenance, acquisition of materials, increase of the staff and improvements in the computerized system; and specific, related to the management process, with actions aimed at improving the local work process. Conclusion: The description of the problems for patient safety in Primary Care makes it possible to list strategies aimed at strengthening the safety culture.

Método: Investigación descriptiva, con abordaje cuantitativo y cualitativo. En el cuantitativo se utilizó el instrumento Encuesta de Consultorio Médico sobre Cultura de Seguridad del Paciente y en el cualitativo se eligió el modelo de cadena causal y la Teoría del Desarrollo Organizacional. La recolección de datos tuvo lugar de julio a diciembre de 2017, en 14 unidades de salud de una ciudad del sur de Brasil. Participaron 51 enfermeras. Resultados: Problemas de seguridad del paciente relacionados con: equipo, exámenes, registros médicos, cantidad profesional, prisa, flujo de trabajo, gestión del servicio, siendo la cantidad más importante que la calidad, el intercambio de información y a los profesionales que creen que los errores pueden usarse en su contra. Para ello, se desarrollaron estrategias genéricas, relacionadas con la estructura, como mantenimiento de equipos, adquisición de materiales, aumento de personal y mejoras en el sistema informático; y específicos, relacionados con el proceso de gestión, con acciones orientadas a mejorar el proceso de trabajo local. Conclusión: La descripción de los problemas para la seguridad del paciente en Atención Primaria permite enumerar estrategias encaminadas a fortalecer la cultura de seguridad.Objetivos: Descrever os principais problemas relacionados à segurança do paciente na Atenção Primária à Saúde na perspectiva dos enfermeiros; elencar estratégias de mudança que fortaleçam a segurança do paciente na Atenção Primária à Saúde. Método: Pesquisa descritiva, de abordagem quanti-qualitativa. Na quantitativa utilizou-se o instrumento Medical Office Survey on Patient Safety Culture e na qualitativa optou-se pelo modelo de cadeia causal e a Teoria do Desenvolvimento Organizacional. A coleta de dados ocorreu de julho a dezembro de 2017, em 14 unidades de saúde de uma cidade do sul do Brasil. Participaram 51 enfermeiros. Resultados: Descreveram-se problemas de segurança do paciente relacionados: aos equipamentos, exames, prontuários, quantitativo profissional, à pressa, ao fluxo de trabalho, à gestão do serviço, à quantidade ser mais importante do que a qualidade, à troca de informação e aos profissionais que acreditam que os erros podem ser usados contra si. Diante destes, foram elaboradas estratégias genéricas, relacionadas à estrutura, como manutenção de equipamentos, aquisição de materiais, aumento do quadro de pessoal e melhorias no sistema informatizado; e e específicas, relacionadas ao processo de gestão, com ações voltadas a melhoria do processo de trabalho local.
