Removal of intraradicular pin followed by endodontic reintervention of elements 14 and 15: Case report

Resumo: Removal of intraradicular retainers often becomes an obstacle when endodontic retreatment is required. The aim of this study was to describe an endodontic reintervention preceded by the removal of intraradicular posts using the Integrated Simultaneous Ultrasound System (SISU) technique. Female patient, feoderma, 34 years old, was referred for dental care at the Specialization Clinic in Endodontics, Faculdade São Leopoldo Mandic, Belo Horizonte Unit, with indication for endodontic reintervention of dental elements 14 and 15, both with the presence of intraradicular post. In the initial radiographic examination, it was possible to observe a slight thickening of the periodontal ligament in tooth 14 and the presence of a radiolucent image in the region adjacent to the root apex, suggestive of a periapical lesion in tooth 15. After confirming the need and feasibility of endodontic retreatment, removal was performed. of the prosthetic crown, followed by partial wear of the cast metal cores and removal of the cement line with a fine tip ultrasonic insert. After this procedure, two ultrasonic thick tip inserts were used, positioned simultaneously in opposite directions, so that the ultrasonic vibrations were transferred to the intraradicular retainers, thus allowing their removal. Endodontic retreatment was performed in two sessions, with intracanal medication for a period of 21 days. Patient under clinical and radiographic follow-up.

El objetivo de este estudio fue describir una reintervención endodóntica precedida por la remoción de postes intrarradiculares utilizando la técnica Integrated Simultaneous Ultrasound System (SISU). Paciente femenina, feoderma, 34 años, fue remitida para atención odontológica en la Clínica de Especialización en Endodoncia, Facultad São Leopoldo Mandic, Unidad de Belo Horizonte, con indicación para reintervención endodóntica de los elementos dentarios 14 y 15, ambos con presencia de poste intrarradicular. En el examen radiográfico inicial se pudo observar un ligero engrosamiento del ligamento periodontal en el diente 14 y la presencia de una imagen radiolúcida en la región adyacente al ápice radicular, sugestiva de lesión periapical en el diente 15. Luego de confirmar la necesidad y la factibilidad del retratamiento endodóntico, se realizó la extracción de la corona protésica, seguido por el desgaste parcial de los núcleos metálicos fundidos y la extracción de la línea de cemento con un inserto ultrasónico de punta fina. Después de este procedimiento, se utilizaron dos insertos ultrasónicos de punta gruesa, colocados simultáneamente en direcciones opuestas, de modo que las vibraciones ultrasónicas fueran transferidas a los retenedores intrarradiculares, permitiendo así su remoción. El retratamiento endodóntico se realizó en dos sesiones, con medicación intracanal por un período de 21 días. Paciente en seguimiento clínico y radiográfico.A remoção de retentores intrarradiculares muitas vezes torna-se um obstáculo, quando há necessidade do retratamento endodôntico. O objetivo desse estudo foi descrever uma reintervenção endodôntica precedida de remoção de pinos intrarradiculares utilizando a técnica Sistema Integrado e Simultâneo de Ultrassom (SISU). Paciente do sexo feminino, feoderma, 34 anos, foi encaminhada para atendimento odontológico na clínica de Especialização em Endodontia, da Faculdade São Leopoldo Mandic, Unidade Belo Horizonte, com indicação de reintervenção endodôntica dos elementos dentários 14 e 15, ambos com presença de pino intrarradiculares. No exame radiográfico inicial, foi possível observar ligeiro espessamento do ligamento periodontal no dente 14 e presença de imagem radiolúcida na região adjacente ao ápice radicular, sugestiva de lesão periapical no dente 15. Após confirmada a necessidade e viabilidade do retratamento endodôntico, foi realizado a remoção da coroa protética, seguido do desgaste parcial dos núcleos metálicos fundidos e remoção da linha de cimentação com um inserto ultrassônico de ponta fina. Realizado esse procedimento, foram utilizados dois insertos ultrassônicos de ponta grossa, posicionados simultaneamente em direções opostas, de forma que as vibrações ultrassônicas fossem transferidas aos retentores intrarradiculares, permitindo, dessa forma, a remoção dos mesmos. Foi realizado o retratamento endodôntico em duas sessões, com medicação intracanal por um período de 21 dias.
