Assessment of the relationship between gastroesophageal reflux disease, smoking and dental wear: narrative review of the literature

Resumo: | The objective of this study was to evaluate the relationship between gastroesophageal reflux disease (GERD), smoking and tooth wear. A narrative review was conducted, performing an electronic literature search using the PubMed/Medline platform. Cross-sectional, cohort and case- control studies carried out in adults were included, addressing at least two of the three research topics. Animal studies, case reports, literature reviews, book chapters, letters, commentaries, editorials, and in vitro studies were excluded. There were no restrictions regarding language and publication date. The search resulted in 523 articles, 46 of them met the inclusion criteria. A total of 156,843 people from 20 countries participated in the selected studies. Most articles found a significant association between smoking and the occurrence of GERD, which can be explained by mechanisms involving Lower Esophageal Sphincter (LES) relaxation, reduced salivary secretion and increase in gastric emptying time. In addition, individuals with GERD may have reduced pH in the oral cavity, which can lead to degradation of dental structures, a process that can be exacerbated by the direct impact of smoking. The results of this study suggest that there is a positive relationship between GERD, smoking and tooth wear. However, further prospective studies evaluating the interaction of these three factors are needed to corroborate these findings. |

Se realizó una revisión narrativa, realizando una búsqueda bibliográfica electrónica utilizando la plataforma PubMed/Medline. Se incluyeron estudios transversales, de cohortes y casos y controles realizados en adultos, que abordaran al menos dos de los tres ejes de investigación. Se excluyeron estudios en animales, informes de casos, revisiones de literatura, capítulos de libros, cartas, comentarios, editoriales y estudios in vitro. No hubo restricciones con respecto al idioma y la fecha de publicación. La búsqueda arrojó 523 artículos, de los cuales 46 cumplieron con los criterios de inclusión. Un total de 156.843 personas de 20 países participaron en los estudios seleccionados. La mayoría de los artículos encontraron una asociación significativa entre el tabaquismo y la aparición de ERGE, que puede explicarse por mecanismos que implican la relajación del esfínter esofágico inferior (EEI), la reducción de la secreción salival y el aumento del tiempo de vaciado gástrico. Ademas, las personas con ERGE pueden sufrir episodios de reducción del pH en la cavidad bucal, lo que promueve la degradación de las estructuras dentales, proceso que puede verse exacerbado por el impacto directo del tabaquismo. Los resultados de este estudio sugieren que existe una relación positiva entre ERGE, tabaquismo y desgaste dental. Sin embargo, se necesitan más estudios prospectivos que evalúen la interacción de estos tres factores para corroborar estos hallazgos.O objetivo deste estudo foi avaliar a relação entre a doença do refluxo gastroesofágico (DRGE), tabagismo e desgastes dentais. Uma revisão narrativa foi conduzida, realizando-se uma busca eletrônica na literatura, através da plataforma PubMed/Medline. Foram incluídos estudos transversais, de coorte e caso-controle realizados em adultos, abordando, pelo menos, dois dos três temas foco da pesquisa. Estudos em animais, relatos de caso, revisões da literatura, capítulos de livros, cartas, comentários, editoriais e estudos in vitro foram excluídos. Não houve restrições com relação ao idioma e à data de publicação. A pesquisa resultou em 523 artigos, sendo que 46 deles atendiam aos critérios de inclusão. Participaram dos estudos selecionados um total de 156.843 pessoas distribuídas em 20 países. A maior parte dos artigos encontrou uma associação significativa entre o tabagismo e a ocorrência de DRGE, o que pode ser explicado por mecanismos envolvendo o relaxamento do Esfíncter Esofágico Inferior (EEI), a redução da secreção salivar e o aumento do tempo de esvaziamento gástrico. Além disso, indivíduos com DRGE podem sofrer com episódios de redução do pH na cavidade oral, promovendo a degradação das estruturas dentais, processo que pode ser exacerbado pelo impacto direto do tabagismo. Os resultados deste estudo sugerem que há uma relação positiva entre DRGE, tabagismo e desgastes dentais.
