Resumo: The objective of this study was to describe the mechanisms used by nursing students to promote health and prevent diseases to hypertensive and diabetic patients assisted by a Family Health Strategy team, based on a problematic teaching and learning methodology. This is an experience report of nursing students from the Federal University of Piauí during the course of Curricular Internship I when performing an intervention project in a UBS in the city of Picos-Piauí. It was used to develop the invention project the Problematization Methodology, following the steps proposed by Charles Maguerez. There was a social action of HyperDia, individual consultations and group educational activity, involving more than one hundred patients. The objectives were achieved, making it clear that the importance of nurses is linked to the process of health education, motivating hypertensive and diabetic patients to realize awareness and self-care, knowing the great resistance of adherence to treatment, providing a systematized care with intention. to improve the quality of care, requiring more specific knowledge, planning, organizing and developing with the team individual and collective actions aimed at the promotion, prevention and recovery of the health of these patients.

Este es un informe de experiência de estudiantes de enfermaria de la Universidad Federal de Piauí durante el curso de Pasantía Curricular I al realizar un proyecto de intervención en una UBS en la ciudad de Picos-Piauí. Se utilizó para desarrollar el proyecto de invención de la Metodología de problematización, siguiendo los pasos propuestos por Charles Maguerez. Hubo una acción social de HyperDia, consultas individuales y actividad educativa grupal, que involucró a más de cien pacientes. Los objetivos se lograron, dejando em claro que la importância de las enfermeiras está vinculada al proceso de educación en salud, motivando a los pacientes hipertensos y diabéticos a darse cuenta y cuidarse a sí mismos, conociendo la gran resistência de la adherencia al tratamiento, brindando una, atención sistematizada com intención. Para mejorar la calidad de la atención, requiriendo um conocimiento más específico, planificación, organización y desarrollo con el equipo de acciones individuales y colectivas dirigidas a la promoción, prevención y recuperación de la salud de estos pacientes.Objetivou descrever os mecanismos utilizados por acadêmicos de enfermagem para promoção da saúde e prevenção de agravos à hipertensos e diabéticos assistidos por uma equipe da Estratégia de Saúde da Família, a partir de uma metodologia problematizadora de ensino e aprendizagem. Trata-se de um relato de experiência vivenciado por acadêmicos de enfermagem da Universidade Federal do Piauí durante a disciplina de Estágio Curricular I ao realizarem um projeto de intervenção em uma UBS do município de Picos-Piauí. Foi utilizada para desenvolver o projeto de invenção a Metodologia da Problematização, seguindo as etapas propostas por Charles Maguerez. Realizou-se uma ação social de HiperDia, consultas individuais e atividade educativa em grupo, contemplando mais de cem pacientes.
