Clinical management of newborn with cyanotic heart disease: a bibliographic review

Resumo: Congenital heart diseases (CHD) represent an important cause of morbidity and mortality, accounting for approximately 8% of infant mortality. Of these, approximately 30% of deaths occur in the early neonatal period. They are defined as abnormalities in the cardiocirculatory structure and function, present from birth, caused by changes in the embryonic development of the heart. They may occur due to genetic, environmental or idiopathic factors and are classified as cyanotic and acyanotic. This present study aims to carry out a narrative literature review on the clinical management of cyanotic congenital heart diseases. Articles from the Pubmed, Scielo and Google Scholar platforms were analyzed, using descriptors such as: congenital heart disease, cyanosis and newborn. The studies evaluated showed the importance of early diagnosis of cyanotic CHD, which can be performed during pregnancy, through fetal or even postnatal echocardiography. It was observed that from the suspicion or confirmation of this comorbidity, it is essential to institute the appropriate clinical management of this newborn. Because, when treatment is instituted early, it has contributed to increase the survival of these children.

Se definen como anomalías en la estructura y función cardiocirculatoria, presentes desde el nacimiento, causadas por cambios en el desarrollo embrionario del corazón. Pueden ocurrir debido a factores genéticos, ambientales o idiopáticos y se clasifican en cianóticos y acianóticos. El presente estudio tiene como objetivo realizar una revisión narrativa de la literatura sobre el manejo clínico de las cardiopatías congénitas cianóticas. Se analizaron artículos de las plataformas Pubmed, Scielo y Google Scholar, utilizando descriptores como: cardiopatía congénita, cianosis y recién nacido. Los estudios evaluados demostraron la importancia del diagnóstico precoz de la CC cianótica, que puede realizarse durante el embarazo, a través de ecocardiografía fetal o incluso posnatal. Se observó que a partir de la sospecha o confirmación de esta comorbilidad, es fundamental instituir el manejo clínico adecuado de este recién nacido. Porque, cuando el tratamiento se instaura de forma precoz, ha contribuido a aumentar la supervivencia de estos niños.As cardiopatias congênitas (CC) representam causa importante de morbimortalidade, sendo responsáveis por cerca de 8% da mortalidade infantil. Destes, aproximadamente 30% dos óbitos ocorrem no período neonatal precoce. São definidas como anormalidades na estrutura e na função cardiocirculatória, presentes desde o nascimento, causadas pela alteração no desenvolvimento embrionário do coração. Podem ocorrer por fatores genéticos, ambientais ou idiopáticos e são classificadas como cianóticas e acianóticas. Este presente estudo tem como objetivo realizar uma revisão de literatura narrativa sobre o manejo clínico das cardiopatias congênitas cianóticas. Foram analisados artigos das plataformas Pubmed, Scielo e Google Scholar, utilizando descritores como: cardiopatia congênita, cianose e recém-nascido. Os trabalhos avaliados evidenciaram a importância do diagnóstico precoce das CC cianóticas, podendo ser realizados na fase gestacional, através da ecocardiografia fetal ou até mesmo pós-natal. Observou-se que a partir da suspeita ou confirmação desta comorbidade, é imprescindível a instituição do manejo clínico adequado desse recém-nascido.
