Primary anorectal melanoma: a case report

Resumo: Primary mucosal melanoma is a rare condition, corresponding to 1.4% of melanomas in general, being more common in the anorectal region due to the higher concentration of melanocytes. It is more prevalent between the sixth and eighth decade of life. It presents symptoms such as: local pain, enterorrhagia, hematochezia, presence of anal tumor mass, evacuation discomfort, which can generate prolapse during defecation due to the growth of the lesion. In face of vague and nonspecific signs and symptoms, the disease is characterized by having an unfavorable prognosis, as its diagnosis is usually late, being made in only 0.9 to 5% of cases, usually, due to the presence of complications. The identification of the disease is confirmed through histopathology. The present study describes the clinical case of a 69-year-old patient, who underwent, in the face of complaints, a polypectomy with biopsy that presented an intraepithelial malignancy of the anal canal with free margins, characterizing the procedure as a dressing.

Es más frecuente entre la sexta y octava década de la vida. Presenta síntomas como: dolor local, enterorragia, hematoquecia, presencia de masa tumoral anal, molestias al evacuar, que pueden generar prolapso durante la defecación por el crecimiento de la lesión. Ante signos y síntomas vagos e inespecíficos, la enfermedad se caracteriza por tener un pronóstico desfavorable, ya que su diagnóstico suele ser tardío, realizándose solo en el 0,9 al 5% de los casos, habitualmente por la presencia de complicaciones. La identificación de la enfermedad se confirma mediante histopatología. El presente estudio describe el caso clínico de una paciente de 69 años, a la que se le realizó, ante las quejas, una polipectomía con biopsia que presentó una malignidad intraepitelial del canal anal con márgenes libres, caracterizando el procedimiento como aderezo.O melanoma primário de mucosas é uma afecção rara, correspondendo a 1,4% dos melanomas no geral, sendo mais comum na região anorretal devido a maior concentração de melanócitos. Tem maior prevalência entre a sexta e oitava década de vida. Apresenta sintomas como: dor local, enterorragia, hematoquezia, presença de massa tumoral anal, desconforto evacuatório, podendo gerar prolapso durante o ato defecatório devido ao crescimento da lesão. Diante de sinais e sintomas vagos e inespecífico, a doença é caracterizada por ter um prognóstico desfavorável, pois seu diagnóstico geralmente é tardio, sendo feito em apenas 0,9 a 5% dos casos, normalmente, mediante à presença de complicações. A identificação da doença é confirmada através do histopatológico.
