Social violence generates (pseudo) hypertensive crisis: the view of the user of primary health care

Resumo: Violence is present in today's society and seems to contribute to the occurrence of hypertensive crises. The aim of this study is to describe the perception of the user of primary health care about this association (violence and hypertensive crises). Descriptive study carried out with hypertensive adults in the area enrolled in a Primary Health Care Unit in Fortaleza-Ceará, with semi-structured interviews in the year 2018. The reports were transcribed in full and Bardin's Categorical Analysis was carried out. Four units of meaning were defined: 1- perceived feelings and hypertensive crises; 2- violence and hypertensive crises; 3- family conflict and hypertensive crises; 4- change in the social context and hypertensive crises. The findings showed that when hypertensive residents of violent areas witness and experience situations of violence, this triggers the crisis, sometimes perceived by different symptoms, sometimes by checking blood pressure. It is concluded that many of the cases were actually hypertensive pseudocrisis and that the distinction between them requires the skill of the multidisciplinary team, as they are at odds in the approach.

El objetivo de este studio fue describer la percepción de los usuarios de la atención primaria de salud sobre esta asociación (violencia y crisis hipertensiva). Estudio descriptive realizado con adultos hipertensos del área inscritos en una Unidad de Atención Primaria de Salud en Fortaleza-Ceará, con entrevistas semiestructuradas en el año 2018. Se transcribieron los informes íntegramente y se realize el Análisis Categórico de Bardin. Se definieron cuatro unidades de significado: 1- sentimientos percibidos y crisis hipertensiva; 2- violencia y crisis hipertensiva; 3- conflict familiar y crisis hipertensiva; 4- cambio en el context social y crisis hipertensiva. Los hallazgos mostraron que cuando los residents hipertensos de áreas violentas presenciam y viven situaciones de violencia, esto desencadena la crsis, a veces percibida por diferentes síntomas, a veces por control de la presión arterial. Se concluye que muchos de los casos fueron en realidad pseudocrisis hipertensiva y que la distinción entre ellos require la habilidad del equipo multidiscplinario, ya que están reñidos en el abordaje.A violência está presente na sociedade atual e parece contribuir para a ocorrência de crises hipertensivas. O objetivo deste estudo foi descrever a percepção do usuário da atenção primária em saúde sobre essa associação (violência e crise hipertensiva). Estudo descritivo desenvolvido junto a hipertensos adultos da área adscrita de uma Unidade de Atenção Primária à Saúde de Fortaleza-Ceará, com entrevistas semiestruturadas no ano de 2018. Os relatos foram transcritos na íntegra e realizada Análise Categorial de Bardin. Foram definidas quatro unidades de significado: 1- sentimentos percebidos e crise hipertensiva; 2- violência e crise hipertensiva; 3- conflito familiar e crise hipertensiva; 4- mudança no contexto social e crise hipertensiva. Os achados mostraram que, quando hipertensos moradores de áreas violentas presenciam e vivenciam situações de violências, isso desencadeia a crise, algumas vezes percebidas por sintomas diversos, outras pela verificação da pressão arterial.
